Triangle of Care is a set of standards that ensure family, friends and carers are included and supported by Health and social care services.
Kent and Medway NHS and Social Care Partnership Trust (KMPT) has joined the Triangle of Care membership scheme and are working to improve our services and commitment to carers. To do this we want to knowwhat we are doing well, and where we need to improve, in our working with and supporting carers.
