New Patients
Patients wishing to register with the Practice should collect the necessary forms from reception. The forms are also available online, to print off or email when completed. If you wish to pre-register please visit our website. When you register you will also be asked to fill out a medical questionnaire. There is an online version of this file too, which you may fill out and send to us. Alternatively, you may print off a registration form, fill it out and bring it to the practice to register. When the forms are completed please return to them to surgery with appropriate photo ID and a letter that confirms your current address.
All new patients over the age of 40 or are on repeat medication are invited to attend the surgery within a month of joining to have a general health check with the practice nurse. Please try to bring details of past illness, vaccinations and smear history, where applicable, and bring a sample of urine for testing. If you are on any repeat medication, please also make an appointment to see a doctor.
Named Accountable GP
All patients are assigned an accountable GP. Once you are registered if you wish to know who your accountable Doctor is, or you have a preference then please contact the practice, or ask when you are next in the surgery. Your accountable GP will take responsibility for your overall care at the practice, however this does not affect your right to see any doctor or nurse of your choice. Please note if you need to be seen in an emergency on the day we will allocate you an appointment with any available doctor.
We are pleased to announce that we had our Care Quality Commission audit in April 2017 and the surgery has been given a Good overall rating in all areas
Internet Services
We have developed a new range of services for you to use over the internet. You can now book or cancel an appointment remotely by registering for our online services - forms are available at surgery. You will also have access to update your contact details and personal information with the links provided. Once you have created an account you will also be able to request repeat medication, however for your security we will require identification to activate this service. We are currently updating our online services and will be able to offer access to you medical record in the near future. Prescriptions can also be ordered via email – .
New Clinical System
The practice in conjunction with Bolton CCG will be upgrading their clinical system as from the end of July 2018; when we are upgrading our clinical systems we will be only be able to offer urgent appointments for the first week as we will have no access to your record.
Patient Advisory Group
Would you like to have a say about the services provided at Dr Jain and Dr Subramanian. The Patient Advisory Group would like to hear your views. By providing your email details we can add them to a contact list that will mean we can contact you by email every now and again to ask you a question or two. Please visit email the surgery () or ask a receptionist for a form.
Complaints & Comments
Any general comments, compliment or complaint about the practice should be made to the Practice Manager in writing; forms are available via our website (drjainanddrgupta.nhs.uk) and from our reception desk.
See a Doctor or Nurse through the week and at Weekend for Routine Appointments
A new service is being introduced in Bolton to give patients more flexibility in where, when and how they use local health services. Patients registered with a GP in Bolton can now book appointments to see a doctor or nurse at weekends at the following surgeries:-
