PRC program is available to Carroll County Residents. Income Guidelines do apply to this program. You may contact Carroll County DJFS at 330-627-2571 to inquire of the income guidelines for your household.

PRC Program Requirements:

Must have at least 1 of the following to apply:

* Minor child(ren) in the household

* Pregnant

* Current Child Support Order in Carroll County

PRC Program can help with:

* Disconnection/ Terminations of Utilities (ex. Electric, gas, Propane, Water)

* Eviction/ Foreclosure Notice for housing

* New Employment Services (ex. Gas to go to work, Testing required to work, Uniforms required for work

* Vehicle Repairs

PRC Verifications needed for each as stated below:- Subject to change (If you do not have the verifications needed, please ask DJFS staff what your options may be)

1.  Disconnection/ Terminations of Utilities (ex. Electric, gas, Propane, Water)

a.  Previous 30 days of earned and unearned (SSI, Child Support, etc) Income for all adults in household

b.  Current Bank Statement with name, account, and balance

c.  Picture ID/ Driver’s License

d.  Disconnection/ Termination notice

e.  Harcatus/ HEAP determination may be required (ask DJFS staff if applicable for you or not)

2.  Eviction/ Foreclosure Notice for housing

a.  Previous 30 days of earned and unearned (SSI, Child Support, etc) Income for all adults in household

b.  Current Bank Statement with name, account, and balance

c.  Picture ID/ Driver’s License

d.  Notarized eviction/ foreclosure notice

3.  New Employment Services

a.  Previous 30 days of earned and unearned (SSI, Child Support, etc) Income for all adults in household

b.  Current Bank Statement with name, account, and balance

c.  Picture ID/ Driver’s License

d.  Fill out New Employment forms (provided at front desk of DJFS)

e.  Employer to verify working at least 20 hours per week, hourly rate, and hire date in writing

f.  If applying for gas to get back and forth to work, we need in addition to the following:

i.  Work Schedule

ii.  Notarized License affidavit (can obtain at DJFS front desk)

iii.  Valid Driver’s License of person driving to the workplace

4.  Vehicle Repairs

a.  Previous 30 days of earned and unearned (SSI, Child Support, etc) Income for all adults in household

b.  Current Bank Statement with name, account, and balance

c.  Valid Driver’s License

d.  Employer to verify working at least 20 hours per week and hourly rate by either pay stubs, or in writing.

e.  2 estimates for the repairs

f.  Proof of Insurance

g.  Title(s) of all vehicles in household

h.  Notarized License Affidavit (can obtain at DJFS front desk )
