Please Read and then sign each of the statements found on the following page.

1. Required Materials:

These materials are DUE BY MONDAY, AUGUST 31st and will be kept IN CLASS.

  • Composition note book
  • pencil
  • A folder with 3 holes that can be used to bring homework back and forth and to store completed work.
  • A paper bag that will be used to cover their text books.

2. Due Dates and Late Work, Assignments and Assessments:

  1. Assignments that are given when a student is absent must be made up. It is the responsibility of the student to find out what work has been missed. If a student knows in advance that he/she will be missing class for vacation, school activity, etc. They need to get the assignment in advance in order to keep caught up.
  2. There is an online student text with tutorial materials to assist students with specific questions.

C. ALEKS Assignments:

  • This year we will be using ALEKS, an on-line mathematics program. Students will be required to complete homework assignments on this program on a regular basis. Students will have the ability to attempt the assignment as many times as they desire before the due date. Each student’s highest score will be recorded.

D. Assessments:

  • All work must be shown for full credit to be given.
  • A practice test or quiz will be given before each assessment so that students are aware of what to expect on the assessment.
  • There will be no retakes on tests or quizzes.

3. Grading :

A 94 and above; A- 90-93;

B+ 87-89; B 80-86; B- 78-79;

C+ 75-77; C 64-74; C- 60-63;

D+ 58-59; D 52-57; D- 50-51; F 0-49

4. Textbooks:

Students are responsible for keeping their texts in the best possible condition. Students must keep their books covered with a paper bag or fines will be assessed at the end of the year. Participation points will be given periodically throughout the year for having their book covered. If their book is not covered on the day of the book check, students will be able to cover their book for partial credit. Fabric book covers are NOT acceptable. They should keep their papers in a binder rather than stuffing them in their books.

5. Citizenship:

  • It should be understood that the policies and procedures of the school and district will be strictly enforced, and that daily attendance will definitely have a positive impact on success in this class.
  • Tardies: 3 tardies – N; 5 tardies – U; 6 tardies – administration referral.
  • I consider talking during instruction time, note writing, unpreparedness, disobedience, fighting, destruction of property and texting during class time bad manners and poor citizenship.
  • I will notify parents if I feel that citizenship is an issue. I will not change citizenship grade to N or U until parents have been notified.

7. General Information

A. Access to grades is available to parents and students via portal. They are updated weekly.

B. Seating charts will be used for roll-taking purposes. If students are not in their assigned seat, they will be marked absent.

C. The math department is available to help any math student from a designated math teacher.

E. Extra credit will be given for signed progress reports and boxtops.

F. The Math Department would like to make use of the technology available to us to make this the best possible experience for your child this year. To do so we will be digitally recording lessons and posting them on-line. Students will also be participating in lessons by presenting their work to the class. Because of this, we ask that you sign the release allowing South Ogden Jr. High to post your child’s image on blogs and Weber Tube. We may also occasionally post photos and videos of some Math activities just for fun.

Please know that we understand there are circumstances under which you may not want your child’s image on-line. You are in no way obligated to agree to have your child’s image on our blog or Weber Tube.

G. Accessing homework either through the teacher’s blog or a link to a website may be asked of your student. If you do nothave access to the internet and a printer, please let the teacher know so arrangements can be made to produce a copy for your student.

H. Electronic devices will be allowed only with the approval of your teacher.

I understand that a poor citizenship grade in any class can affect my child’s ability to perform in extracurricular activities including cheer, student government, field trips, and sports. I understand that you will call or email me if there is a problem with my child’s behavior that could result in a change in citizenship. I understand that poor citizenship includes the following behaviors: improper use of language, improper use of cellphones, tardiness, uncooperativeness, endangering others, destroying property, nonparticipation, cheating or plagiarism, and disobedience.
