If you believe that you have any legal basis to show why this action is unjustified, you may request an Administrative Show Cause Hearing, at which time any argument may be presented to a hearing officer for review. A $25.00 fee is required when requesting a Show Cause Hearing. A request for a Show Cause Hearing does not postpone the time in which to file a writ of certiorari in accordance with s. 322.31 F.S.

This order has been filed in the official records of the Division of Motorist Services on August 1, 2013. Appeals may be initiated within 30 days of the date of this order by following the procedures specified in s. 322.31 F.S.

HOW TO CLEAR THIS SUSPENSION

To reinstate this suspension before August 21, 2013:

1.  Contact the Polk County Court thru the contact information on the front of this letter and meet their requirements.

2.  The court will need to give you an order/affidavit indicating that you have met their requirements. The order/affidavit must have a court seal or judge’s signature and indicate that you complied with their requirements before August 21, 2013. The affidavit/order is only valid for 30 days.

3.  Take the order/affidavit to your local driver license office or a tax collector office that offers driver license services. There will be a $6.25 fee if you choose to use the tax collector office. You can also mail the order/affidavit to one of the below addresses. If you mail the order/affidavit, please allow 7-10 business days for processing.

To reinstate this suspension on or after August 21, 2013:
