WHO WE ARE

Family Connection Partnership is a public/private partnership created by the State of Georgiaand private funders. We assist communities in addressing the serious challenges facing Georgia’s children and families. We also serve as a resource to state agencies across Georgia that work to improve the conditions of children and families.

Working jointly with Family Connection community collaboratives, we are implementing a vision for community-based support systems that will build a better future for our communities.

SERVICES:

  • The Helpline is a 24-hour telephone support program that offers specialized information about Alzheimer’s disease, chapter services, community resources, emotional support, referrals, and follow-up counseling services. There is also a Helpline On-Line available to families and caregivers.

  • Care Consultation provides advice, problem-solving counsel, decision-making support, crisis assistance, guidance and referrals to families/caregivers who have made contact with the Alzheimer’s Association. Care Consultation is a group of services that assist in planning for and coping with all aspects of the disease experience including assessment of needs, assistance with planning and problem-solving, and provision of support.
  • Support groups for caregivers and families are lead by trained professionals and volunteers who address concerns and provide answers to the day-to-day challenges of caring for someone with Alzheimer’s disease. Support groups are offered monthly at various sites and times as well as a regularly scheduled telephone support group, known as the Caring Connection.
  • A nationwide identification program that works at the community level to provide assistance for individuals who become lost or have wandered from their home.
  • The Caring Closet provides monetary reimbursement to caregivers for incontinence supplies (adult briefs, creams, wipes, bed pads and gloves) on a monthly basis.
  • When we think about staying fit, we generally think from the neck down. But brain health plays a critical role in almost everything we do. This program focuses on things we can do to keep our brain healthier as we age and steps that may help reduce our risk of Alzheimer’s.
  • The Family education series consists of workshops on a variety of topics including an overview of Alzheimer’s disease, communication, caregiver stress, and legal issues.
  • Targets those in the healthcare field. Trainings provide information, support, and assistance on a variety of topics designed to improve the caregivers understanding of the disease and skills needed to care for people with the disease. Professionals can also receive a certification in Alzheimer’s and dementia care through our Alzheimer’s Training Institute (ATI).
  • The lending library provides caregivers with books & videos on a wide range of caregiving topics. The website (alz.org) provides a wealth of information on the disease process and caregiving concerns.

ELIGIBILITY: Children referred to our program must be of school age (ages 5-18), have guardian permission to participate in the program, and must want and need a mentor to succeed in life. Priority is given to serve the children in our areas of intentionality – children of prisoners and children at risk of truancy or dropping out of school. Youth referred by schools may be served with high school or college students for specific programs to enhance the educational achievement of both the mentor and the mentee.

SERVICES: Big Brothers Big Sisters of the Heart of Georgia provides one-on-one mentoring for school aged children in 15 Central Georgia counties.

SERVICES: Bright From the Start oversees a wide range of programs focused primarily on children ages birth to school age and their families. These include:

  • Administering the nationaly recognized Georgia Pre-K Program

  • Offers quality used and new merchandise at a very affordable price. Items are free in certain circumstances.
  • Monetary assistance for electricity, propane, rent, mortgage, food, and medical needs is available for those in need. Usually only 1 time per year for each household.
  • Currently sponsoring a Monroe County mobile food drop and the Monroe County Food Resource Pantry.

  • Free educational programs about topics such as identity theft or credit management to local schools, civic, and church groups.
