MIV Data Entry Guide

General Information

For the initial conversion to MIV, it is recommended that all Biography Form Annual Supplement forms for the period under review be consolidated onto one Biography Form Annual Supplement form prior to converting to MIV. It has been found to be significantly more difficultto try and consolidate the information during the cut/paste conversion process.

Each activity should be a new entry. For activities extending over a oneyear period, extend the year rather than writing another duplicate entry.

Place dates for all items at the beginning of the entry. MIV will store all data added over the years in one continuous list. By making dates easily visible at the beginning of each record, itwill simplify the submission process in the future when candidates are selecting records to include in a review period.

Data Entry

All data entry is completed under the “Enter Data” tab in MIV. The majority of the data entry for librarians will be completed under the category “Service: Additional Information.” The information below is for how to enterdata into this Category.

Select “Enter Data” then “Service” then “Additional Information”

Four headings have been added under the Service: Additional Information category to match the MOU Categories. When adding a new record, use the “Add a New Record” button under the appropriate heading. Do not add a new section or edit/delete the existing section headings.


When adding a new record, a blank text box will appear for cutting/pasting or direct typing. It will show the section to which the record is being adding. Formatting options are available, but some may not display the same way on the PDF that is generated, especially in regards to indents and spacing.

Make sure to “Save” the record when complete.

In the Additional Information category of MIV, all new records added to a section will be automatically placed at the bottom of the section. To resequence items, forrearranging in date order or by order of importance, select the resequence button.

Drag and drop items within a section by holding down the mouse button over the record until you get the four-arrow cursor.

Resequencing of records can only occur within each Section. If you need to move a record from one section to another, you will need to copy the text into a new record in the desired section and delete the record from the old section.

Make sure to “Save Resequencing” when completed, or cancel if necessary.

You can add, edit or delete records, but do not add, edit or delete the Section headings.

Managing Packets (formerly Dossier)

Creating a packet is the first step to being able to view and select data to include with the packet to be submitted.

Select “My Packets” then “Manage My Packets”then the “Add Packet” button.

A box will pop-up, asking you to Name the Packet. The packet for this demonstration was named “2015 Review”

The packet will be created and you will be taken to the screen where you can select the data to be added or removed from your packet.

When working with the information added into the Service: Additional Information category, selected the +to the left of “List of Service” to expand this category.

If additional/supplemental information was added into other categories, such as Publications, Honors and Awards, or Grants and Contracts, they may be selected/expanded as well to include/exclude records for the review period. Following the instructions below.

This is the screen where boxes can be checked/unchecked for any records that should be included in or excludedfrom the current review packet. This will become more important in future review cycles where information from previous reviews will already exist in MIV. Do not uncheck the boxes or edit headings for the main heading “Biography Form: Annual Supplement” or any of the 4section headings. Even if no information is included in a section, the heading should still appear in the PDF so that reviewing bodies know it was not forgotten, the same as when using the paper Biography Form Annual Supplement. Save your Design!

Preview your Packet

Return to the Manage My Packets screen.

To preview the packet as a PDF, select the Preview link.

This step simply generates the PDF for viewing and printing. You can view a pdf of your entire packet, or individual sections of your packet.

Edit your Packet

Return to the Manage My Packets screen. When you place your mouse over the area where your packet is listed, you will then see options for Edit, Delete, Duplicate, Rename.

When you selected Edit, you will be returned to the screen which allows you to add or remove items from your packet.

Submitting your Packet

Packets are no longer submitted directly by the candidate after completion, but are submitted after a Packet Request has been initiated by Library’s Academic Personne Coordinator (Debbie Snapp).

After the Packet Request has been initiated, you will see a red 1 by My Packet Requests (in place of the green 0 in the example below.
