This form is pursuant to the requirements set forth in G.9-6 and G.21-2D.1(e) which state that the call of Ministers serving out of bounds must be reviews and renewed annually. The purpose of the annual review is to enable Presbytery to (a) evidence its concern for the ministry being performed; (b) to outline presbytery’s expectations of the minister; (c) where appropriate, to consult with the calling agency as an advocate for the minister; and (d) provide Presbytery with information to determine if your status should be renewed for another year. NOTE: All information shared on this form is confidential and for Ministerial Committee use only.
