Substitute Teacher Invoice Form

It has been the policy of the Milwaukee Mathematics Partnership (MMP) to reimburse schools for substitute teachers when they are needed for a MMP activity. This reimbursement occurs electronically after the MMP receives a correctly completed Substitute Teacher Invoice Form. All substitutes should first be paid out of school funds. Then schools will submit a completed Substitute Teacher Invoice Form to Henry Kranendonk. Forms should be submitted once a month. The following guidelines have been written in order to improve the submission of Substitute Teacher Invoice Form and to clarify the procedure that needs to be followed to assure correct reimbursement.

Name of Substitute: A requirement in order to process the form.

Name of Teacher covered by the substitute: A requirement in order to process the form.

MMP Activity: Activities should clearly described. MMP activities include but are not limited to: (1) monthly MTL meetings or Assessment pilot meetings, (2) School-based Action Plan or Assessment Action Plan activities, (3) Learning Team meetings when math is involved. Activities need to be explained briefly but specifically.

Date of Activity: A requirement in order to process the form.

Substitute Hours: The MMP will reimburse schools for substitute teachers under the following circumstances: (1) full day coverage of a classroom, (2) internal coverage of classrooms-hourly rate, (3) Sage teacher coverage. Reimbursement includes the rate of $189.92 + benefits (full day) or $23.74 + benefits (per hour of internal coverage). Please mark the Substitute Teacher Invoice Form using one of these three terms (full day, number of hours, or SAGE)

MMP Office: For office use only no need to complete.

MMP Substitute Teacher Request Form: MTLs should complete this form and turn it in to the head secretary. The information on this form should assist the secretary in filling out the Substitute Teacher Invoice Form. All MTL request forms should be attached to the completed Substitute Teacher Invoice Form when submitted for reimbursement. All invoices should be submitted to Henry Kranendonk, Mathematics Curriculum Specialist, Department of Teaching and Learning, Central Services, Rm 253. Fax 475-8074 or 475-8457.

Notification of Transfer: Each school will receive a confirmation letter from the MMP indicating that the electronic transfer has been completed. Please contact Terry Burbey, 475-8468 if you have questions or have not received notification of the transfer.
