Milieu’s Home Sharing Services provides supports to Adults with developmental disabilities that wish to share a home with someone who provides ongoing support. Milieu’s Home Sharing Support Model is a broad and flexible service that includes a wide variety of support and living arrangements. In some situations, home sharing involves very close relationships. The individuals within the home share not only share their personal space, but, also their lives. The members of the home spend a lot of time together and are actively involved in one another’s daily activities. In other situations, the home sharing arrangement is characterized by more independent relationships. The members of the home generally go their own ways and come together at specific times or for specific purposes.

However, despite the endless variations, there are some commonalities within all of these arrangements. Those who are contracted to provide home sharing have the following responsibilities:

Treat the individual with respect and dignity at all times

Fully integrate and treat the individual as an equal participant in the home and community

Ensure the individual’s safety and general well-being

Provide support and guidance

Offer nutritious meals that respect cultural backgrounds and dietary needs

Provide positive adult role modeling

Create an atmosphere that fosters independence and personal growth while ensuring the individual remains safe

Ensure the individual is provided with appropriate medical and dental care and follow routines prescribed by health professional

Nurture positive relationships between the individual and members of his/her family and support network

Encourage and support the individual to connect with community and to access generic services

Milieu provides each contracted caregiver with a complete handbook outlining the contractor’s obligations with respect to, the above notes responsibilities. This handbook is reviewed and signed off annually at during their contract renewal process. A Home Sharing Survey was established to include caregivers in the survey process. This year 10 surveys were given out, and 7 were returned. This is down from last year’s returns of 90% to 70%.

The Annual Home Sharing Review is a user-friendly, time efficient tool that allows the caregiver to perform a self analysis and then review with the Home Sharing Coordinator. This tool can and is used to review our Home Sharing Services on a placement by placement basis.

Home Sharing Services Changes

Over the past year, there have been some changes to our demographics. Unfortunately, one of the individual’s in home sharing passed away. We also had an individual move and therefore, have to leave our service. There was also one new individual added into our services. They are all included in the demographics above.

Home Sharing Survey Results

Home Sharing Services has rapidly expanded over the past few years and Milieu felt it was important to include caregivers in the survey process. The following is a breakdown of Home Sharing survey questions and responses.

Outcome: Caregivers are satisfied with the level of contact from the Home Sharing Coordinator?

Action Plan: The Home Sharing Providers report that the Home Sharing coordinator’s attendance at 2 meetings or more in a year, as well as phone calls and emails have increased their satisfaction with the level of contact they receive from the Home Sharing Coordinator.

Complete:Yes, this was a question on the Home Sharing Survey. Of the survey’s returned, 100% said they were satisfied with the level of contact that they had with their Home Sharing Coordinator compared to 11% in the previous year.

Home Sharing Services Effectiveness

Outcome: Home Sharing caregivers report that continuing education training once per year has increased the effectiveness of their service delivery.

Action Plan: Milieu updated their Home Sharing Provider Contracts this year. Home Sharing Providers are now expected to attend at least one continuing education course a year.

Complete:Yes, 100% of returned surveys reported that the Home Sharing Providers felt that participating in continuing education once per year increased their effectiveness as a Home Sharing Provider.

Home Sharing Services Efficiency

Outcome: Home Sharing Caregivers report efficiency in the time spent submitting their on-line monthly reporting through the use of Sharevision.

Action Plan: Each month, Home Sharing Providers are expected to submit a monthly report on ShareVision. Once every three months, they are expected to fill out and participate in an emergency drill. The Home Sharing Coordinator will support the Home Sharing Providers with any ShareVision questions they may have.

Complete:Yes, 100% of the surveys returned, said that the Home Sharing Providers had received enough technical support and training to input their reports as well as felt that their time spent submitting these reports were efficient.

Home Sharing Services Accessibility

Outcome: Caregivers are satisfied with the accessibility of Milieu information about meetings and events to Home Sharing Providers.

Action Plan: The Home Sharing Coordinator focused on making Agency information available to Home Sharing Providers this year. This included information on contractual obligations, ShareVision, and Agency changes.

Complete:Yes. 100% of returned surveys felt that they were provided with useful information and were adequately informed about issues affecting their work.

Home Sharing 2010-2011 Strategic Plan

Satisfaction

Outcome:Increase the opportunities for the Home Sharing Providers to connect.

Action Plan: Home Sharing Providers made note on the Survey that they did not feel they had many opportunities to connect with other Home Sharing Providers. The Home Sharing Coordinator will make sure that they are invited to the Family Forums quarterly as well as the Family Picnic in the fall.

Effectiveness

Outcome: Maximize the Home Sharing Provider’s effectiveness by giving them an updated Home Sharing Provider Handbook outlining their roles and responsibilities.

Action Plan:The Home Sharing Coordinator will update the Home Sharing Provider’s Handbook and give a copy to each Home Sharing Provider so that they fully understand their role.

Efficiency

Outcome:Decrease the amount of time it takes to screen a new Home Sharing Provider by increasing the efficiency of screening tools.

Action Plan: The Home Sharing Coordinator has recently taken a Home Sharing Screening Course and will be

updating all of the Agency’s information in order to make the screening process more efficient.

Accessibility

Outcome: Maximize the accessibility of ShareVision to parents of individuals supported through Home Sharing.

Action Plan: The Home Sharing Coordinator will give access to parents/family of individuals supported through Home Sharing to ShareVision. ShareVision documents how goals are going and has other important information about the individuals. Parents will also be able to access Agency information such as Annual Reports and dates for Family Forums.
