MIER/MSISR/GIMS

PURPOSE

This practice provides the standard method that must be used by all P2S entities and locations – office, self-perform, and construction management/subcontracting; joint venture (JV) and limit liability companies (LLC) – in compiling and submitting HSE data relating to occupationally-related injuries, illnesses, fires, motor vehicle accidents, spills of hazardous materials; also, HSE citations, near-miss events, and chemical exposures.

APPLICATION

This practice applies to work activities and employees under the control of P2S and its contractors.

DEFINITIONS

GIMS – Global Information Management System; the central computer database managed by P2S HSE for collecting, storing, collating, and reporting HSE-related data from P2S-managed entities.

JV – Joint Venture; companies retain individual legal liabilities although collectively all companies of JV have legal requirement to do 100 percent of the work.

LLC – Limited Liability Company; a completely new company entity; P2S shares ownership with other companies in the LLC. Determining “operational control” becomes unclear since it is a new company typically with employees from multiple companies working together side-by-side as LLC employees. If majority/plurality owner of LLC, then P2S will report in HSE statistics. LLC, like a JV, has responsibility for reporting and tabulating exposure hours and injuries independent of the owner company’s reporting protocols.

MIER – Monthly Incident Experience Report; the standard report of HSE-related events pertaining to a P2S-managed entity.

MSISR – Monthly Subcontractor Incident Summary Report; the standard report of HSE-related events pertaining to subcontractors and their lower-tier contractors on a P2S-managed project or office site.

1.0PROJECT/OFFICE STARTUP

When a contract is awarded where P2S is a part, the HSE Project Incident Reporting Sheet (Form 000.653.F0309) will be completed by the Business Unit/Line HSE staff and presented to P2S Corporate HSE for concurrence and approval. Refer to Attachment01 for a discussion on JVs and LLCs.

2.0GENERAL REQUIREMENTS

2.1Project Incident Log

HSE Representatives in all locations (offices and sites) will maintain either a Project Incident Log (Form 000.653.F0193 or equal) and provide information on all HSE-related events up the “line” or HSE management chains as requested, or a “project incident list” in GIMS (refer to Attachment02 for an example).

HSE Representative or designee responsibilities include:

  • Accumulate data relative to occupationally-related HSE event.
  • Adhere to cutoff for monthly reporting, which is 12 midnight the last Saturday of each month.
  • Know what first aidis (refer to Practice 000.653.1202, Attachment01).
  • Distribute copies of monthly performance reports as prescribed by the project and/or Business Unit HSE Lead.

3.0MIER

The MIER is developed electronically from several “components” in GIMS for P2S offices, self-perform, construction management/subcontract, JV, and LLC entities (refer to Attachment01 for an example).

4.0MSISR

There is no MSISR within GIMS and there is no report as such. In order to accomplish the same results, contractors will need to be entered individually — then a Subcontractor Detail report must be run to get the monthly breakdown that previously showed on the MSISR.

5.0GIMS

Information pertaining to occupationally-related injuries and illnesses will be entered into GIMS on a real-time basis (as soon as practical) following the incident.

A “hard copy” of the MIER and MSISR should be printed from GIMS:

  • A copy scanned and emailed/faxed to Sheila Ballard (refer to Attachment02 for an example.
  • A copy retained in the project HSE files.
