Signature and Title Page Templates

Step 1

Find the appropriate set of title and signature pages for your document.

Master’s thesis (MS students, thesis option): pages 2 and 3
Master’s report (MS and MFor students, report option): pages 4 and 5
Dissertation (PhD students): pages 6 and 7

Step 2

Update all information highlighted in yellow.

Note that the degree program does not always match the administrative home department or school.
Your home department or school can also be found on the form TDR-Review, which was given to you after the Graduate School reviewed your draft document.
Match the case and style of elements. If something is in all capital letters, keep it in all capital letters, for example.
The committee listing uses the paragraph style “Signature Line” to create the aligning tabs and spacing between the lines. See our blog if you need assistance to edit this style.

Step 3

Remove the header text on each page and highlighting from all text.

Step 4

Print the desired pages to PDF, using the “High Quality Print” or “Press Quality” conversion settings. Insert the pages into your final dissertation, thesis, or report.
