Sestamibi Parathyroid Scan

Indications/History: Elevated serum hormone levels

Findings: This examination was performed with image acquisition of the neck and mediastinum both early and with delayed image acquisition after the intravenous administration of 27.1 mCi Tc99m Cardiolite. On the early images, there is mild asymmetric uptake of the submandibular glands with normal activity seen in the right side and diminished on the left side. The parotid glands demonstrate normal increased uptake. There is markedly increased uptake in the left side of the thyroid bed. The right side of the glad shows normal size with relatively vague or diminished uptake.

Subsequent delayed imaging demonstrates that there is persistent focal increased uptake in the left side of the thyroid bed. The recent ultrasound exam from 1-20-17 did not demonstrate a definite abnormality in this location. However, the intensity of uptake should be regarded with a high degree of suspicion for parathyroid adenoma.

Impression: There is intense focal early and delayed uptake within the distribution of the left thyroid lobe. The findings are highly suggestive of a parathyroid adenoma.
