MEDICAL TREATMENT AND LIABILITY RELEASE

I, the undersigned parent/guardian, do hereby grant permission for my child,, to participate in cheerleading/workouts conducted at Trenton HighSchoolKid’s Camp. Instruction will be conducted by THS Cheerleaders and Coaches. In order that my child may receive the necessary medical treatment in the event of illness or injury, I hereby hold the Trenton cheerleaders and coaches harmless in the exercise of this authority.

I further acknowledge, understand and agree that in taking part in this activity, there is a possibility of physical illness or injury and that my child is assuming the risk of such by participation. I further agree to hold harmless Trenton Public Schools, their associates, staff, administration, coaches and cheerleaders, for any illness or injury by my child during the cheerleading lessons/workouts.

In case of emergency, I hereby give permission to transport my child to the nearest hospital for emergency treatment. I recognize that as a result of participation, medical treatment on an emergency basis may be necessary and further recognize that personnel may be unable to contact me for my consent for emergency medical care, including hospital care, as may be deemed necessary under existing circumstances. I understand that I will be contacted as soon as possible and will be advised to any treatment by the hospital doctor.

The Trenton Coaching Staff and Fall 2016 Trenton Cheerleaders would like to present to your current or future cheerleader the opportunity to attend the 15th Annual Trenton Cheerleading Skills Camp. The camp will be held at your athlete’s elementary school.

Thursdays, Sept 29th – Oct 20th3:30 p.m. – 5:00 p.m.

This camp is open to girls that are in Kindergarten – 5th grade in the fall of 2016. The cost of the camp is $60.00. Registrations are due by September 22nd to Coach Maci at Trenton High School to guarantee a t-shirt and spot at camp.

The $60.00 camp fee will include:

  • Group instruction by current Trenton coaches and cheerleaders (Participants will be grouped according to age and each group will be led by Trenton cheerleaders.)
  • T-Shirt in their size (please specify and payment must be made by Sept 22nd)
  • The chance to learn what it means to be a competitive cheerleader; learn basic skills, cheers, chants, a dance, and much more…
  • Have the opportunity to cheer at a Trenton High School Football game.

*Please send your athlete with a snack and water bottle each week.
