Position Summary

This position is a clinical pharmacist position in a base hospital, fulfilling roles that include:

  • Medication reconciliation
  • Clinical ward pharmacy
  • Dispensing
  • Provision of medication-related advice to clinical staff and patients
  • Commitment to provision of a high quality, safe, efficient and effective pharmacy service ensuring continuum of patient care
Selection Criteria

Essential:

  • Bachelor of pharmacy degree
  • Pharmacist registration with Australian Health Practitioner Regulation Authority (AHPRA)
  • Commitment to improving patient outcome through medication management
  • Membership to an appropriate professional body- Society of Hospital Pharmacists of Australia or Pharmaceutical Society of Australia
  • Hospital Pharmacy experience

Desirable:

  • Experience in using iSOFT software
  • Commitment to Continuing Education
  • Excellent time management, organisational, communication and team building skills
KeyWorking Relationships

Internal:

This position reports to the Director of Pharmacy under the Medical Division at Wimmera Health Care Group. This position liaises with pharmacy department staff, medical and nursing staff, and with allied health and administrative staff.

External:

This position liaises with community pharmacy staff, staff from small rural hospitals and pharmacy departments within larger hospitals. They may also interact with other professional and government bodies such as Medicare programs, Pharmaceutical Society of Australia, VicTAG, Drugs and Poison Unit, DHS, from time to time.

Responsibilities & Performance Indicators

Service Delivery

Responsibilities:
  1. Provision of a clinical pharmacy service to wards on a rotational basis
  1. Perform medication reconciliation
  1. Provision of an appropriate, timely and adequate supply of medications to inpatients, discharge patients and outpatients.
  1. Provision of education to other health care professionals
  1. Promotion and provision of medication information and education to patients
  1. Maintains a level of competency required for the position and Continuing Education hours as per AHPRA guidelines
/ Performance Indicators:
  1. Review prescriptions and medication orders, liaise with medical and nursing staff to ensure safe and appropriate medication use.
  1. A medication reconciliation will be performed on high risk patients within a timely manner as outlined under WHCG policy
  1. Maintain drug inventory control, in consultation with other staff, on all stocks of restricted drugs on the wards through imprest, recording of date and stock rotation. Supervision of the dispensing and the dispensing (and claiming) of PBS items, dialysis, SAS and chemotherapy. Manufacture and supervision of the manufacture of non sterile items when required. Preparation of and supervision of preparation of Webster packs for appropriate patients
  1. Participate in and when required develop and present staff education sessions. Provision of a drug information service to the hospital and to smaller regional centres
  1. Supplying information, both written and verbal on all new discharge drugs relating to indications of use, safety, possible side effects of the drug, how the drug works and compliance. Supplying medication profile summaries. Perform workshops such as cardiac rehabilitationand pulmonary rehabilitation
  1. Demonstrate evidence of ongoing pharmacy education via the collation of Continuing Professional Development points as per AHPRA requirements. Attends all mandatory training sessions as deemed necessary for the position. Keep up to date with all new pharmacy services, medications and improvements to keep in line with best current practice

  7. Function in accordance with legislation, conducting practice within a professional and ethical framework to deliver care that protects the rights of individuals and groups.
  8. Collaborate with multidisciplinary team members to achieve desired health outcomes for patients.
  9. Assume accountability and responsibility for providing a high standard of direct patient care through assessment, planning, implementation and evaluation of outcomes.
  10. Practice within policy and procedural guidelines.
  11. To prescribe, dispense or administer medications according to WHCG medication policies, procedures and guidelines; and within professional requirements.
  12. Responsible for preventing and managing pressure injuries according to WHCG Pressure Injury Prevention policy and procedures.
  13. Responsible for preventing and managing falls according to WHCG Falls and Falls Injury Prevention policy and procedures.
  14. Responsible for the safe management of equipment – this includes using equipment within standard operating guidelines, conducting appropriate preventative maintenance and not using unsafe equipment.
  15. Responsible for the reporting of malfunctioning equipment and/or facility to the appropriate line manager.
  16. Responds to Emergency Medical Reviews and Code Blue’s as per the Code Blue Response Procedure and Emergency Medical Review Policy.

Training & Development

Responsibilities:
  1. Completes annual mandatory training requirements as per organisational policies.
  2. Recognises the need for ongoing commitment to personal and professional development.
  3. Maintains a level of competency required for the position.

Occupational Health & Safety (OH&S)

Responsibilities:
  1. Ensures compliance with Occupational Health and Safety and Workcover legislation and regulations.
  2. Maintains current knowledge of WHCG emergency procedures.
  3. Carries out duties in a manner which does not adversely affect their own health and safety or that or others.
  4. Cooperate with measures introduced in the interest of health and safety.
  5. Undertake any training provided in relation to Occupational Health and Safety.
  6. Immediately report all matters that may affect workplace health and safety to their Manager/Supervisor.
  7. Report all incidents, near misses or injuries as soon as practicable after the event.
  8. Correctly use any information, training, personal protective equipment and safety equipment provided by WHCG.
  9. Refrain from recklessly or wilfully interfering with anything that has been provided for health and safety reasons.
  10. Participate in emergency response drills and attend relevant emergency training.
  1. Seek clarification on any issues relating to tasks and previous trainings that may affect health and safety withinWHCG.
