PRESCRIPTIONS
You should have 2 days medication left before ordering prescriptions, as it takes 48 working hours/ 2 WORKING DAYS for it to be signed and ready. We CANNOT accept requests over the telephone. All requests MUST be ordered in person at the surgery OR via your local pharmacy.

HEALTH CHECKS
Patients aged 40-74 will be invited for a health check. The health check involves:
- Measuring your blood pressure
- Measuring your height and weight
- A simple blood test to measure your cholesterol and sugar levels
- Finding out about your family history
SCREENING PROGRAMMES
There are programmes for the following:
- Women’s Cervical Cytology
- Breast screening
- Bowel screening
CARERS
Do you look after someone or does someone look after you?
Evidence shows that if the carer becomes ill, the likelihood is that the patient cared for is affected in somewhere.
Please ask at the reception for a Carers Form so that we can record your details.
For Carers that are registered at this practice please can you book an appointment for an annual Carers Review with the Reception Team.

SMOKING AND ALCOHOL STATUS
Please inform the reception team of your Smoking and Alcohol status so that we can update your records. We can offer support if required, please do not hesitate to ask.

Every year around 100,000 people die from smoking, with many more deaths caused by smoking-related illnesses.
