MCES SST PROCEDURES CHECKLIST

Tier 1- Best Practices for Classroom Instruction (Academic AND Behavioral) by all Teachers with the end goal of instruction meeting the need of 80% of all students

_____ Monthly PLC Meeting where the four questions are the focus and data used to drive instruction.

ñ  What do we expect our students to learn?

ñ  How will we know they've learned it?

ñ  What will we do if they haven't learned it?

ñ  What will we do if they have already learned it?

*PLC Meetings must be documented*

_____ Differentiation is a daily vital component of Tier I

ñ  This is what we do for all students based on a need

ñ  We use formative assessments/ per-assessments to determine the differentiation

_____ Intervention/Enrichment Block

ñ  Data must be used to determine skills/strategies/pre-teaching/enrichment groups

ñ  Use progress monitoring when appropriate

_____ For students not grasping concepts in Tier I, start Tier 2 process

Tier 2 – Classroom instruction with differentiation is not meeting the student's needs or the student has

scored 1 or 2 on the EOG

______Evaluate effectiveness of intervention using progress monitoring data

ñ  Is the progress less than grade level/goal expectation

ñ  Determine if intervention is working but needs increased intensity/duration

_____ Write a SMART Goal to address the problem

ñ  Document SMART goal on Tiered paperwork

ñ  Access SMART goal weekly through progress monitoring

_____ Student will be added to next PLC grade level meeting agenda

ñ  Teacher will bring tiered paperwork with SMART Goal and progress monitoring data

ñ  Grade level team will help determine if there is a need for change in intervention, intensity, and/or duration

______Parent Conference with teacher and/or team

______For students not making Tier 2 progress after a minimum of four progress monitoring data

points, return to PLC grade level team for determination of next steps

_____ After no less than two grade level PLC meetings, start Tier 3 process and continue tiered

paperwork

Tier 3 – SST Process (while continuing previous tiered interventions)

_____ Teacher submits Tiered Paperwork to SST Chairperson at Introductory SST meeting for student

_____ Scheduling the meeting

ñ  SST Chairperson schedules meeting

ñ  Classroom teacher notifies parent using Parent Notification Letter/Invitations and submits copy to SST Chairperson during initial meeting

ñ  Teacher will also send social history form with invitation and ask for form to be returned before first formal meeting

_____ At first formal SST meeting

ñ  Review of interventions and data from Tier 1 & 2 (include EC Chairperson)

ñ  Questions/clarifications from social history

ñ  Interventions are brainstormed

ñ  Interventions are formalized and written as SMART goals

ñ  Intervention fidelity observation and student observation is scheduled and assigned to one SST member

ñ  Date and time for follow up meeting is set

______At second and consecutive follow up meetings

ñ  Evaluate intervention, observation, and data from established SMART Goal

ñ  Discuss additional SMART Goals or next steps

Tier 4 – EC referral
