  • Seller will provide any and all health records for this dog and guarantees this dog to be in good health at the time of adoption.
  • Buyer agrees to provide a safe and loving home and environment for this dog INCLUDING that under NO circumstance this dog is allowed to be outside running without a leash or in an unconfined area. Annual visits to the veterinarian will be made to maintain vaccinations and rabies.
  • Buyer agrees that this will be an INSIDE ONLY dog and will not be left outside for extended periods of time.
  • This dog has been kept current on Heartworm and flea preventative. Buy also agrees to keep this dog on heartworm/flea preventative for the duration of his/her life. Note: The dog will need to be taken to the buyer’s vet within 30 days and tested for heartworm in order to get a prescription for heartworm medication. If by some chance the dog comes back heartworm positive, please notify the seller and the dog can be returned to the seller for a full refund if desired.
  • Buyer agrees to assure quality of life for this dog, to love, care for, and nurture him or her as a treasured member of the buyer’s family.
  • If at any time the buyer cannot care for this dog, the seller must be notified and the dog returned to the seller. UNDER NO CIRCUMSTANCE is this dog to be placed in a pet shop, animal shelter or similar establishment. Also if the buyer cannot care for this dog and wishes to pass it onto family or a family friend, the seller must be notified, the dog returned to the seller, and the seller must be able to meet the person/persons the dog is going to live with. It is then at the sellers discretion if the person/persons are able to care for the dog. Any breech of this will result in legal actions. ______(initials of buyer)

Current Feeding Information

This dog is currently on Blue Buffalo Wilderness Adult (Chicken).

He/she is eating ______cups per day and this amount is divided into a morning feeding and an evening feeding.

Buyer agrees to all terms and conditions and realizes that failure to follow said terms and conditions will void this agreement.
