I will come to a Re-Visit session on ____/____ during ______.

I plan to retake the exam on ____/____ during ______.

·  If you receive 70% or below on a test, you MUST re-take it! If you receive above a 70% you may OPTIONALLY re-take it. These are the rules…**

You must attend one “Re-Visit” session within 5 school days of test return

§  You must schedule a time for this session with your teacher. If your teacher is not available during your SOAR, then you have to meet with your teacher before or after school or attend the Algebra Clinic Club (see “Clubs” on the ERMS web site for further information) – your teacher will let you know their preferred time for re-visits.

§  Students are responsible for making an appointment for the re-visit; teachers will not remind or coax you to do this.

§  Part of the re-visit work includes correcting incorrect problems on the original test on a separate piece of paper (do not change the original test!), which includes, for each incorrect question, showing all work as well as providing a written explanation of what caused the error, and how the error will be avoided in the future.

Re-testing must take place by the deadline set by the teacher. Individual teachers can determine mode of re-test. Possibilities include:

§  Re-test with new questions

§  Re-test by working one-on-one with a teacher demonstrating mastery by working the problem without help and either orally or in writing explain what they are doing

§  Teacher may choose another method at their discretion

§  Students must show ALL WORK. Re-takes will not be accepted with just an “answer”

§  Parent signature must be on the original test

§  Re-tests must be done at school

Students earn full credit back for correct responses.

·  Students who MUST re-take a test and DO NOT follow the steps above by the deadline, will be reported to their dean to complete the assignment during Friday or Saturday school, or at a time the dean designates.

** These are the minimum requirements for the retake policy. The teachers reserve the right to waive and/or apply additional conditions if deemed appropriate on a case by case basis.
