TC-27r(2) RECERTIFICATION ADJUSTMENT NOTICE

TERMINATION OF ASSISTANCE

This form is used as a second “notice” to call a resident into the Management office for an interview when they have not responded to the TC-27r “Recertification Adjustment Notice” within the specified time listed on the form. The TC-27r(2) form is the final notice to raise the resident’s rent to Market Rate rent for non-compliance with paragraph 16 in the Rental Agreement.

These instructions should be followed:

1.  Ensure that the property name, apartment number, resident(s) name(s), and the date are filled in.

2.  Record the original date the TC-27r “Notice” was delivered to the resident. Then, fill in the appropriate Market Rate rent for the unit and the date Market Rate rent will apply – 10 days from the date listed on this “Termination of Assistance” form.

3.  Be sure to enter the Property’s telephone number where indicated. Then Owner/Owner’s Agent must sign the “notice”. If additional comments or instructions are needed, please enter them on the lines provided.

4.  Please make two photocopies of the original notice and deliver to the household as follows: original – by 1st class mail; one photocopy – hand delivered; and one photocopy – placed in the resident file in the “Certification” section for reference.

5.  Please record the appropriate information in the “Office Use Only” section of the form. Be sure to record the date and time of delivery.

6.  When the resident(s) respond(s) to the “notice”, please complete an “Interview” (form TC-59a). This Interview should include a reminder to the resident(s) of the consequences of not reporting all household members and income changes in a timely manner as required by their rental agreement (lease).

7.  If the resident(s) does/do not respond, please see the example below:

September 16: a) Termination of Assistance form TC-27r(2) is delivered

b)  Response required by September 25th

c)  NO Response

October 1: a) Tenant begins paying market rent

b)  Eviction may also be pursued**

**NOTE: Regional Property Supervisors must be informed of all pending evictions.
