Background information:

A co-production approach has being taken to agree the three priorities and action plan. Service user and carers groups were involved at the initial stages to ensure that they were given the opportunity to input their views and to discuss the key themes and what they felt was important. Their feedback and involvement will continue throughout the process

At the six monthly reviews we will monitor our progress in relation to our commitment to making it real and our markers of progress, ensuring that service users, carers and citizens are involved.

We have established individual work streams to support the implementation of the Care Act and we are conscious of how personalisation is embedded throughout the Care Act.

We have signed up to our commitment as a Council and so we are working in partnership to ensure that developments made are fed back throughout the directorates. Adults, Health and Communities working closely together to ensure we are sharing good practice and moving towards our markers of progress.

Information and advice

Trusted information sources are established and maintained that are accurate, free at the point of delivery and linked to local and community information services.

Local advice and support to include user-led organisations, disabled peoples and carers organisations, self- advocacy and peer support

Active and supportive communities

People are supported to access a range of networks, relationships and activities to maximise independence, health and wellbeing and community connections (including public health). Our work in the future with Inclusive Change will further develop community capacity and move towards the next stage of personalisation, creating strong and inclusive communities.

Personal budgets and self- funding

Everyone eligible for ongoing council funded support receives a personal budget. Direct payments are the main way of taking a personal budget and good quality information and advice will be available.
