With this in mind PLEASE can you RETURN YOUR COMPLETED APPLICATION FORM with a cheque payable to “MASC” for the correct amount to the address AT THE TOP OF THE FORM NO LATER THAN Sunday 29th June 2014. You can also e-mail us your completed form to BUT ONLY IF YOU CHOOSE TO PAY US ONLINE! You can do this by paying the correct amount to MASC, A/C No. 65234471, sortcode 08-92-99, reference YOUR NAME. If you choose this method please advise us when you e-mail the form.
