LUCY ADDISON MIDDLE SCHOOL
Educator and community leader Lucy Addison was born during the Civil War and educated during the Reconstruction Era of the American south, in Fauquier County, Virginia. After graduating from the Institute for Colored Youth in Philadelphia, she attended Virginia Normal School in Petersburg, Virginia, Cook County Normal School in Chicago, Illinois, and Howard University in Washington, DC.
Miss Addison taught in Roanoke at Gregory, Gilmer and Gainsboro schools, and was named principal of the new Harrison School in 1917. She served Roanoke City Schools for over 40 years until her retirement in 1927. In 1928, a second high school for black students was opened and named Lucy Addison High School. Lucy Addison Middle School reopened fall semester 1999 as a state-of-the-art facility that includes a spacious atrium, deluxe planetarium, four computer labs, a technology lab, Space Center and television studio with closed-circuit TV throughout the building.
The faculty and staff at Lucy Addison Middle School strive to continue Miss Addison’s tradition of academic excellence and service to the community by encouraging students to take their rightful place on the educational journey and to make choices that will determine a stellar destiny. ACCOUNTING PROCEDURES
Welcome to Lucy Addison Middle School. We are so happy to be working with each one of you. It is our hope that we can have many successful years together. If you have any concerns or questions about any of the information contained in this packet, please let the bookkeeper, Lynn Williamson, know. If you are a new teacher and feel you need “extra help”, please schedule a meeting with her. If you have any suggestions for next year's teacher accounting information packet, please let the bookkeeper know.
You will be given a packet of what you need in the way of forms to start off your year. Any form that requires completing (such as the emergency card), please do so and return to the bookkeeper. You will receive the following at the beginning of the year:

EMERGENCY CARDS
Please return your completed emergency card to Ms. Barnette by close of business on the firstday of school. It will remain in a safe, confidential location in the bookkeeper’s office. The card will only be used in an emergency situation should you require emergency treatment. Staffphone numbers or addresses are NOT given to parents or students.
REMITTANCE FORMS
Remittance forms are the most important forms you will submit to the office. They are an accurate account of monies received by you in the classroom. It is mandatory that remittance forms be completed according to instructions. Please follow the following directions:
- Bring money to officein the money envelopes provided to you daily! Teachers are not permitted to hold money in their rooms until the next day. Money must be deposited daily! Also, do not send money to the office by a student! This is unacceptable and may cause an issue with continued employment!
- Fill out accurately and completely. List each student and amount of money given to you. Please remember to give each student a receipt of any money received from them for any purpose throughout the year. Save receipt books so they can be handed into the bookkeeper at year-end.
- Date deposited and date money received should match exactly.
- List each column correctly and specify what money is for. Field trips should be listed by specific names, such as Science Museum, Richmond, etc. Do not send it in described as just field trip. Make sure grade-level team is on the form.
PURCHASES
Items to be purchased must be approved in advance by the Dr. Johnson. Ordering without approval and without documentation from the bookkeeper that a purchase order has been submitted will mean that you will be personally responsible for the payment. You will also need to request a copy of the vendor’s W-9 form if they are not already in the system. Check with the bookkeeper first.
Complete Purchase Order Attachment and return to bookkeeper for principal’s approval. Include address, phone number and FAX number of vendor. After approval, order will be processed. If using personal money for purchase, please allow time for reimbursement. All checks are now processed in Accounting at Central Office.
Purchases are processed through the warehouse before delivery to the school/teacher.
REQUEST FOR APPROVAL OF EDUCATIONAL FIELD TRIPS
Any field trips form must be discussed with Dr. Johnson and Lynn Williamson. Mrs. Williamson will enter it into Triptracker. Please list all adult chaperones on the bottom of the page. If you are not sure who your additional chaperones will be, write "Six chaperones (or whatever) to be added at a later date". These forms must be sent to the office TWO WEEKS prior to your trip. Please make sure to include on your on-line form what type of transportation will be used and how it will be paid. Also, include your name on the form so we will know who is requesting approval for the trip.
You must include the Aptafund requisition number for transportation as part of the TRIP NAME in Trip Tracker. If you do not include the requisition number for transportation as part of the TRIP NAME, your trip will not be approved.
Once your form has been approved, calculate the overall costs involved as well as the total cost per student. This must be done before you send your field trip notices home. Once you and the bookkeeper arrive at the final cost and cost per student, the Principal must approve the calculations.
FIELD TRIP EMERGENCIES
Although we do not collect fees for insurance anymore, a list of participants must be sent to the office prior to the trip. In the event of an emergency or accident, we must have an accurate list of every person on that particular trip. Please list chaperones on the bottom, since they will change from trip to trip. Please make a copy of your list, post it for the elective teachers and keep a copy for your records. A copy of this list and parent permission forms should been taken on the field trip. If students are not attending, a list must be made of all who remain in the building as well as their schedule for that day.
FIELD TRIPS WITH EXPENSES
Purchase Orders and Quote Forms – Always complete the purchase order attachment form and submit it to the office with a bid/quote form if the trip is over $500. Any quote that is over $1,000 must be accompanied by a completed quote form. If the form is not attached, your order will not be processed.If a deposit is required to reserve a bus or rooms, I must have supporting documentation from you and a purchase order attachment form in advance.
Staff must obtain three quotes for expenses, which will exceed $1,000. This applies to all accounts.
Additional approval must be obtained from our business services office before purchases or final arrangements can be made. All expenditures must be accounted for. A receipt must be presented for any money given to you to take on a field trip.
If you need a check to take with you on the day of the trip, please complete the purchase order attachment and submit it to the office two weeks in advance.
FIELD TRIP ESSENTIALS
Criteria – Students must meet the following:
“C” average
No more than five (5) days out or tardies to school
No citizenship grade of a “3”
No more than eight (8) periods in ISS
No OSS
** Students get a fresh start each nine weeks. These criteria must be explained at the beginning of the year.
Letter to Parents – A letter to parents should go home well in advance of the travel date. Submit a copy of your letter to the principal for approval. Your letter should include, but is not limited to, the following information:
- General information about time date and purpose
- How the trip supports the curriculum and/or SOLs
- Who is/is not eligible to participate; be specific to avoid conflict
- A disclaimer about the inability to refund money once it has been paid
Permission Form– The form should specify dates, times, cost, supervision/chaperones, permission for emergency medical assistance.
Chaperones–Male and female chaperones must be present on a field trip involving male and female students.
First Aid Kit/Student Medications– A first aid kit as well as student medications will be prepared for you to take with you on the day of the trip. Give the nurse a list of students attending the trip at least 3 days in advance of the trip so she can prepare necessary medication or information for a field trip. There must be a chaperone who is permitted to administer medication.
School Lunches – Remember to notify Mrs. Cookie, the cafeteria manager, if your class will not be having lunch in the cafeteria or if you need packed lunches.
ACCIDENTS
The faculty and staff shall make a diligent effort to provide a safe environment in the school. Proper supervision in the classroom and throughout the school shall be maintained at all times. If an accident should occur, the following steps must be taken:
- Notify office personnel immediately. Send one student if necessary.
- Proper first aide techniques must be used (preferably using the school nurse, if available).
- The office personnel will notify the parents and/or guardians.
- The office personnel will secure the service of the rescue squad, if needed.
- The principal or the assistant principal will inform appropriate central office personnel if the services of rescue squad or hospital are used.
- An accident report shall be completed for all accidents as soon as possible. See the school nurse or secretary forstudent and employee accident forms.
- Any recorded accident must also be reported to an administrator.
- Employees suffering an “on-the-job” injury are required to seek medical treatment from the Employee Health Clinic located at William Ruffner Middle School.
ASSEMBLIES
Responsibility of the Staff Member:
- Each teacher will be responsible for guiding and seating his/ her students.
- Proper explanation should have been given to all of the students concerning the necessity of good manners and appropriate behavior in the assembly.
- All teachers are to sit with their classes, monitoring and supervising them at all times.
- All teachers will attend the assembly program unless assigned other responsibilities.
- Teachers without a class will sit in the middle of one of the three sections and will maintain proper discipline whenever needed.
General Rules for Students:
- Students should leave the classroom and go immediately to the auditorium/gymnasium under the supervision of the classroom teacher.
- Students will enter the auditorium/gymnasium and proceed to the designated seating area. If in the gymnasium, the students will sit in the same morning designated sections.
- Students may be seated in every other seat as they enter the auditorium.
- As soon as the speaker approaches the microphone, complete attention is expected.
- Students are expected to listen quietly and courteously.
- There will be NO gum chewing, whistling or booing.
- Students are to remain seated at the conclusion of the assembly and remain until dismissed by the principal, assistant principal or the teacher.
STUDENT ATTENDANCE
Attendance and tardies must be recorded in each homeroom electronically and on paper, using pencil. Folders sent to the office after you have entered attendance. Excuse notes must be included in the attendance folder. Prior to marking a student tardy, please check for tardy slip. The attendance folder and computer entries must match. Do not send students to the office for tardy slips when they have been in the building or on the hallway. Students arriving after the attendance has been completed and sent to the office must have a tardy slip from the office to be admitted to class. Because of the time constraints of the rapid notification system, attendance must be entered by 9:00 AM.When the absentee folder is returned, the teacher is responsible for checking to see if the student attendance record is correct for that day. Use UNX or T only. Use present only to make a correction.
Excused Absences
Absences will be excused for field trips (count as present), death in the immediate family, club meetings, athletic events, personal illness, court appearance, and religious holidays. The students must present a note explaining absence from a parent or a guardian to the homeroom teacher following an absence. Students must also present a note from a doctor explaining illness if absent more than three days.
Unexcused Absences
Any absence where there is no written notice from a parent/guardian or staff excusing the student from being absent from school will be unexcused.
Daily absentee reporting and follow-up with the home
Teachers are responsible for keeping daily class attendance records. This is a state requirement and may be subpoenaed by the courts. Any student who is not in class should be recorded as absent (excused or unexcused). Excuse notes from the parent must be sent to the office to be entered.
The first period teacher is designated as the advisor/homeroom teacher. "Housekeeping" tasks should be completed during this time. Attendance should be taken in all classes during the first few minutes of class.
The daily attendance bulletin will be available by 10:30 AM for all staff members. The daily absentee bulletin is confidential and should be used as a reference in reporting absences.
Teachers should contact our Student Support Specialist when unable to reach parents by phone or mail in the case of excessive absences. In reference to truancy issues, teachers please notify the Student Support Specialist of any concerns.
The school will take the following steps with chronic absentee situations:
2 days absent – Phone call home by the Student Support Specialist
5 days absent - Send a letter to the parents explaining the attendance problem and policy
8-10 days absent – The Student Support Specialist will visit the home.
10 days absent–Letter is sent and Student Support Team will meet to discuss the case.
11-20 days absent - Student Support Team will meet to discuss the case and ask for court assistance.
Excusing students from class
Student must have a signed student agenda or a pass to be out of the classroom during class time. Passes should be issued rarely and only when there is a very valid reason.The time of departure and time of arrival should be written on each pass with teacher initials or signatures.If you have an issue with a student or students being frequent flyers to the restroom or hanging out in the hallway, please be sure to alert an administrative team member and contact the parent immediately!
Leaving the school building
Students must check out in the main office before leaving the building. Permission will be given with written parental consent verified by phone call to parent. Upon arrival to school, no student will be allowed to leave the school grounds without parent/guardian permission.
Attendance requirement for after-school activities
Student must attend the regular school session in order to participate in extra-curricularactivities for that day.
