Louisiana Office of Public Health, STD/HIV Program

Purpose of Project: The Louisiana Office of Public Health, STD/HIV Program (SHP) is seeking a qualified agency to implement a Personal Responsibility Education Program targeting African American females ages 14-18 in Region 8 of Louisiana (Monroe metro area). The successful agency will implement SIHLE, a group-level, peer-led intervention designed to reduce HIV risk behaviors among young African American females ages 14-18 in Region 8. Qualifications:

Contractor qualifications include

·  Ability to demonstrate previous experience in coordinating and completing similar projects; effectively reach and recruit the target population into programs; experience successfully implementing evidence based interventions with fidelity; accurate and timely reporting.

·  Must possess the logistical requirements needed to implement the intervention, oversee data collection, and ensure that proper protocols are being uniformly followed.

·  Must either be located in Region 8, or demonstrate existing partnerships/ability to develop partnerships in the Region.

·  Must be in good standing with the Louisiana Secretary of State office and compliant with the Louisiana Legislative Auditor’s office in order to do business in Louisiana and to conduct the activities described.

·  Proposals may be submitted by organizations that are currently funded by SHP, or from new programs/organizations not presently receiving funds from OPH/ SHP.

Budget:

The budget amount will be approximately $74,500 per year for up to 3 years. Budgets will be developed once successful agencies are identified. The contract will have a fee for service structure.

Submission Information:

Deadline for submission is COB Friday, June 14.

All interested applicants must send a proposal of no more than six pages (excluding appendices including relevant CVs and MOUs) doubled space 11 point type, one inch margins, that include a description of:

1.  The agency and its history;

2.  Experience reaching and working with the youth, particularly recruitment and outreach to youth, and successful enrollment and retention in programming;

3.  Experience implementing evidence based interventions, and peer-led small group sessions programs including:

a.  Recruitment logistics and planning

b.  Implementation and fidelity to program model (for evidence-based interventions)

c.  Reporting to and working with funder

d.  Project successes/challenges

4.  Experience in accurate and timely submission of data and reports to funders;

5.  How agency will adhere to the core elements of the SIHLE intervention;

6.  Project implementation plan, including:

a.  Key staff (if already hired, their ability to implement programming, if not, plans to hire) for project

b.  Recruitment strategy (including potential partners)

c.  MOUs with all potential partners

d.  CVs of essential personnel

It is essential that all applicants become familiar with the SIHLE intervention and its core elements prior to submitting this application. Consideration must be given to how the applicant will adhere to all core elements of the intervention, and will implement the intervention with fidelity to the program model.

A technical assistance call will occur on Monday, June 3, 2013 at 1pm. At this time, the Adolescent Health Coordinator will answer questions if needed. Answers to these questions will be posted on HIV411.org within 24 hours following the TA call.
