#10. Authorize various personnel actions as indicated on the summary sheet for employees within the jurisdiction of the Lorain County Board of Commissioners (at the conclusion of today’s Board meeting, the Commissioners may recess into an Executive Session to discuss: personnel/new hires)

#11. Approve & waive reading of the same County Commissioners meeting minutes of October 26, 2011

#12. Accept the Findley Davis actuary reportand establish the Lorain County health care rates and contribution shares for employees forthe year 2012

Community Development Department:

#13. Approve Donald B. Eager & Associates, LLC to provide Fair Housing Services in compliance with CDBGFY11 in the amount of $10,000, effective October 11, 2011 – August 31, 2012

#14. Approve change order #1 to Cross Roads Asphalt, Columbia Station for Texas and Ada Streets, Sheffield Township as recommended by the County Engineer. They will delete the pulvamix roadway and base, aggregate base and asphalt emulsion for a deduction of $30,590 and add cement stabilized subgrate at a cost of $26,547.40, a savings of $4,042.60

Golden Acres:

#15. Approve & enter into a service agreement with Master Textile Management, Cleveland for laundering linens at a cost of $1.99/day/patient. Previous years it was $2.25/day/patient, a savings of $6,000/year. This is effective retroactive to October 1, 2011 – September 30, 2013

Job & Family Services:

#16. Authorize Purchase of Service agreement with Mihalick Maintenance, Elyria for snow plowing/salting services for SYF11/12, effective November 1, 2011 – April 30, 2012 in amount not to exceed $10,000; Authorize Director to execute on behalf of the Board with Prosecutor’s approval as to form and amend for changes in service content and increase value

#17. Authorize maintenance and support services agreement with CGI Technologies and Solution, Inc., for support of applications, software and software updates for the document imaging department, effective January 1, 2012 – December 31, 2012 in amount not to exceed $110,886; Authorize Director to execute on behalf of the Board with Prosecutor’s approval as to form and amend for changes in service content and increase value

#18. Authorize and approve Work Participation Corrective Compliance Plan to meet the federal All Family and the Two Parent Work participation rates in FY12.

Sheriff:

#19. Approve & enter into an agreement with the City of Elyria to lease a portion of their city jail to house the county female population, effective January 1, 2012

1)  #11-043 – Consumers Gas Cooperative, Orrville to bore under Law Rd at house #37700 east of Chamberlain Rd 0.6 mi from south side to north side to install tap and service line, Grafton Twp

2)  #11-044 – Consumers Gas Cooperative, Orrville to bore under Law Rd at house #37684, east of Chamberlain Rd 0.7 mi from south side to north to install tap and service line, Grafton Twp

3)  #11-045 – Columbia Gas of Ohio, Lorain to extend a 4” gas main on north side of Capel Rd starting at 1,347’+- east of Durkee Rd and then extend east 96’+- and cap main at house #37200, Eaton Twp

#7. Lorain County Visitors Bureau Inc., financial statements/quarterly report for period ending September 30, 2011

#8. Mikaela Moore, Midview Middle student collected 840 signatures and 30 letters asking for the Commissioners to reconsider the funding cut to 4H and she also handed out flyers supporting the passage of Issue 22 and had a booth set up at LCCC Family Fun Fest.
