Scholarship for students will be up to $1,000

Vivian Eickoff Scholarship (for Nursing / Health Care Students)
ELEGIBILITY REQUIREMENTS
General
1)Applicant must:
- be a citizen of the United States and Texas residentin theGreater Fort Worth Area.
- be a high school graduate or equivalent.
- have a minimum GPA of 3.0, documented by transcripts from college or high school
Specific
1)Items to be submitted with Application:
- Copies of SAT or ACT scores; college or high school transcripts
- Three letters of recommendation from people other than family members.
- A one page letter describing applicant’s financial need, area of study and action plan to accomplish degree or certification.
2)Completed one of the three attached applications along with any other documentation required by that specific scholarship application. (choose only one of the available scholarships)

METHOD OF DISBURSEMENT
The award will be paid in half-payments at the beginning of each semester and will be paid directly to the institution of learning to be applied towards tuition or other fees in the awardees name. The recipient is asked to comply with the terms as outlined in the Acceptance Form. The award must be used for course work beginning after the award is granted. .
ADDITIONAL INFORMATION
Scholarship(s) will be given on the basis of financial need, achievement, and the criteria of each individual above named Scholarships. Recipient(s) will be notified no later than May 31, 2013 with an invitation to accept their scholarship at a dinner meeting of the organization on June6, 2013.

Ridglea Lone Star TBW will provide dinners for you and one guest. Extra guests will pay $18 for a 3 course meal from Mimi’s party menu. Must have reservations for all guests.

If you choose to accept this scholarship, please acknowledge the following expectations of you:
I will attend the Ridglea Lone Star TBW meeting on June 6, 2013 to accept my Scholarship.
I will maintain a minimum 3.0 GPA.
I will provide an update to the Scholarship Chair after each semester as to my progress and future plans. I realize that the distribution of the remaining half of the scholarship at the beginning of the second semester is contingent upon providing an update.
I understand that I am invited and encouraged to participate in Ridglea Lone Star Texas Business Women functions throughout the year.
