  • Username: Enter yourentireCollin College email address (i.e. )
  • Password: Your default password is your CWID and the last 4 digits of your Social Security Number (total of 13 digits, no spaces or dashes).
  1. After logging in, you should see a list of your full-time staff employees
  • If a full-time staff employee who reports to you is missing from your list OR if an employee is listed who does not report to you, contact Human Resources at 972-985-3783.
  • You ONLY need to complete performance evaluations on full-time staff employees who were hired PRIOR to April 1st. You should complete 90 day evaluations on full-time staff employees hired April 1st or later. Instructions for the 90 day evaluation areemailed to supervisors by Human Resources near the time of the employee’s 90th day of full-time employment.
  • If a full-time employee transferred from another department to your department during the appraisal period, you should complete a regular staff performance evaluation and include feedback from the prior supervisor in order to document performance for the entire year.
  1. Click on the “Details” link to the right of the employee’s name to begin the appraisal

You are not required to complete this document in one sitting. However, in order to save your work and return to the appraisal later, you must select “No” and then click the "Update" button at the bottom of the page.

  • Employee Information - Review the employee information for accuracy. Notify HR of any necessary corrections. Mark the appropriate Yes/No radio buttons indicating whether the employee supervises other employees and if the employee received formal discipline during the appraisal period. These 2 fields MUST be completed before any of your changes will be saved.
  • Employee Self-Evaluation – All employees will complete a self-evaluation and submit it to their supervisor. The supervisor should scanand attach the employee’s completed self-evaluation to the performance appraisal by clicking on the “Browse” button, browsing for the saved file, and clicking “Open” to attach it to the appraisal. Once the file has been successfully uploaded, you will see the file name appear on right of the “Browse” button.
  • Goals -List the employee’s top 3 goals for the performance appraisal period and mark the appropriate radio button indicating whether each goal was Not Completed, Partially Completed, or Completed. Goals listed here should have already been established and discussed with the employee during the appraisal period. If goals were not set with the employee during the appraisal period,you must rate each goal as “N/A” and leave the comments section blank.
  • Achievements - List the employee’s top achievements during the appraisal period.
  • Staff Performance Indicators - After reading the explanation of each staff performance indicator, mark the appropriate rating to indicate whether the employee “Does Not Meet Expectations”, “Meets Expectations”, or “Exceeds Expectations”. Any rating of “Does Not Meet Expectations” OR “Exceeds Expectations” requires supporting comments.
  • Staff Supervisor Performance Indicators - (If the employee does not supervise other employees, this section will not appear.) After reading the explanation of each staff supervisor performance indicator, mark the appropriate rating to indicate whether the employee “Does Not Meet Expectations”, “Meets Expectations”, or “Exceeds Expectations”. Any rating of “Does Not Meet Expectations” OR “Exceeds Expectations” requires supporting comments.
  • Disciplinary Action - (If the employee did not receive aformal disciplinary action during the appraisal period, this section will not appear). Indicate the type of disciplinary action the employee received during the appraisal period and provide comments. Coaching is not considered a type of formal discipline. However, if the employee received a coaching during the appraisal period, the supervisor should indicate that the employee received a coaching and provide comments.
  • Overall Rating - Mark the appropriate overall rating to indicate whether the employee “Does Not Meet Expectations”, “Meets Expectations”, or “Exceeds Expectations”. If the employee received a Level II Disciplinary Warning during the performance appraisal period, the employee must receive an overall rating of “Does Not Meet Expectations”.Enter any overall performance comments in the comments box.
  • Saving an incomplete appraisal - If you are not yet finished with the appraisal and want to save your work, you must select “No” in the “Is the appraisal completed?” section before clicking the “Update” button. You will be able to access the incomplete appraisal again by following steps 1-4 above.
  1. Sign and Date the Appraisal
  • Enter your name and the date you completed the appraisal
  1. Save/Submit the completed appraisal
  • In order to submit your appraisal as complete, you must select “Yes” in the “Is this performance appraisal complete?” section and click the “Update” button at the bottom of the page.
  • After clicking the “Submit” button you should be directed back to your list of employees.

(If you are NOT directed back to your list of employees after clicking “Submit” you have missed a REQUIRED field and your updates will NOT be saved. All ratings of “Does Not Meet Expectations” or “Exceeds Expectations” require supporting comments before marking an appraisal as complete.)

  1. Print the Appraisal
  • After completing and submitting the completed appraisal, you should be automatically directed back to the list of your employees.
  • Click on the “Details” link to the right of the employee’s name and then click on the “Print Performance Appraisal” button on the top right of the appraisal form. (NOTE: You may need to go to “File”, then “Page Setup” on your web browser, and check "Shrink to fit Page Width," so that the entire document will print.)
  1. Meet with the employee
  • After completing and printing the appraisal, you should meet with your employee to discuss the employee’s performance and provide the employee with a copy of the appraisal.
  1. Set goals for upcoming appraisal year
  2. During your appraisal meeting with the employee, you should discuss and set goals for the employee for the upcoming year.
  1. Enter next year’s goals in the performance appraisal system
  2. After setting goals during the appraisal meeting, you should log back into the employee’s appraisal to enter the newly established goals.
  3. Repeat steps 1-4 above to log into the employee’s appraisal. Scroll to the bottom of the appraisal to the question “Would you like to enter next year’s goals now?”
  4. Select “Yes”. This will open 3 fields for you to enter the employee’s goals for the upcoming year.
  1. Allow the appraisal to be viewable by the employee - After meeting with your employee and entering the newly established goals, you mustmake the appraisal viewable to the employee so he or she can access the appraisals to electronically sign the appraisal and enter any comments.
  • Log back into the appraisal form (see steps 1-4 above)
  • Scroll down to the bottom of the appraisal and click “Yes” for “Do you want this performance appraisal to be viewable by the employee at this time?”
  • Click the “Update” button to save your changes.
  1. Send email notification to employee to sign his or her appraisal
  • AFTER clicking the “Update” button, you should be directed back to your list of employees.
  • Click on the hyperlinked text “Send Employee Notification” to notify the employee that his/her performance appraisal is now ready for review and signature.

IMPORTANT NOTE: The word “Yes” should appear under the “Supervisor Complete” and “Viewable by Employee?” columns. If you have not yet marked the appraisal as complete or allowed it to be viewable by the employee s/he will not be able to access or sign the appraisal.

  • Clicking the “Send Employee Notification” link will automatically populate an email in for you to send to the employee. You may edit the email as necessary before sending it.

11. Employee receives email, enters comments and signs appraisal

  • After the manager completes all steps above, the employee will receive an email notification that his/her appraisal is complete and ready for the employee to sign and enter any comments. The email contains a login link and instructions to sign, enter comments and print.
  • Once the employee signs, enters comments, and clicks “Update”, an email notification will be automatically sent to the manager that the employee has signed his/her appraisal. The email will also include any comments entered by the employee.
