Post Title:General Practice Nurse

NHS Education for Scotland General Practice Nursing Career and Development Framework – Level 5

Grade:Depending on experience

Hours:17 per week. A degree of flexibility is required and the ability to work extra hours to cover holidays or sickness or meet the requirements of the service

Responsible to:Professionally: to patients under the NMC Code for nurses and midwives

Managerially: to GPs, Senior Practice Nurses and Practice Manager

Role

  • To provide a comprehensive nursing service as part of a multi-disciplinary primary health care team, within a general practice setting

Introduction

  • Loch Leven Health Centre is situated in Kinross and is a semi-rural medical practice serving over 12,500 patients. We are actively involved in the provision of medical services within the primary healthcare setting. All members of the primary healthcare team work closely together utilising the expert skills of individual members thus providing the best possible care for our patient population.

Accountability

  • The general practice nurse will be wholly accountable for her own actions at all times adhering to the NMC’s Code of Professional Conduct/Scope of Professional Practice and will also be responsible for her own continuing personal and professional development to meet the NMC Revalidation requirements.

Professional

Clinical

  • Provide a high standard of nursing care within the practice premises as requested by the GPs, making accurate observations and reporting as appropriate to the GP or Senior Practice Nurses
  • Under the guidance of the Senior Practice Nurses or GPs, assess nursing need, plan and implement care in partnership with patients and evaluate nursing interventions
  • Provide health promotion and illness prevention services, by undertaking health screening programmes in conjunction with the GPs and the Senior Practice Nurses
  • Practice independently within areas of clinical competence, with reference to Practice Guidelines and Protocols and in accordance with NMC Code of Conduct
  • Identify personal training needs and be responsible for own continuing personal and professional development (utilising clinical supervision networks) in accordance with NMC Revalidation requirements
  • Maintain concise and accurate documentation of care and utilise IT
  • Implements standards for infection control and safeguards Health and Safety of self and others within the performance of her duties

Liaison and Referral

  • Collaborate with the GPs and other members of the Primary Healthcare Team in the care of patients
  • Be an active participant of the integrated nursing team
  • Actively participate in nursing focused meetings for the wider community nursing team, GPs and Practice Manager

Education

  • Identify personal training needs and be responsible for own continuing personal and professional development (utilising clinical supervision networks) in accordance with NMC Revalidation requirements
  • Undertake learning as designated by the practices to ensure effective, evidence based practice.
  • Participate in annual review and maintain a personal learning profile
  • Liaise with Senior Practice Nurses, GPs or Practice Manager to enable the delivery of identified training needs
  • Participate in the training of students of all clinical disciplines
  • Participate in the induction of new members of the primary healthcare team

Clinical Practice Development

  • Be conversant with current research in nursing and update knowledge and skills in order to contribute to the ongoing improvement of clinical and practice nursing expertise
  • Participate in the development of practice protocols and their subsequent implementation and monitoring
  • Actively participate in changes in nursing practice as necessary in light of audit findings
  • Actively participate in setting and maintaining practice standards for practice nursing within the practice

General Clinical Nursing Practice

  • Perform treatment room duties as prescribed by the GPs/Senior Practice Nurses, including : wound care, leg ulcer management, smear taking, immunisations, venepuncture, ear care, urinalysis, ECGs, BP monitoring, minor injuries, assisting with minor surgery procedures
  • Manage emergencies/first aid; assessment by triage; implementation of appropriate emergency/first aid measures, with appropriate referral to GP or hospital A&E
  • Provide appropriate immunisations in accordance with national guidance
  • Provide Health Promotion services working to guidelines developed by the practice team including: cervical cytology, smoking cessation and general health screening

Personnel, Administration, Management

  • Participate in the administrative and professional responsibilities of the practice team
  • Ensure system for record keeping is adhered to accurately
  • To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice

Holiday / 25 days holiday - pro rata for part time staff
10 days public holiday - pro rata for part time staff. 4 days to be taken at Christmas and New Year and one day to be taken at Easter
Sick Pay / During 1st year of serviceOne months’ full pay and (after completing 4 months’ service) one months’ half pay
During the 2nd year of serviceTwo months’ full pay and two months’ half pay
During the 3rd year of serviceThree months’ full pay and three months’ half pay

These payments incorporate entitlements under SSP.
Probationary Period / 12 weeks. Regular reviews will be carried out during the probationary period
Notice Period / 2 years or less service – 4 weeks

Thereafter, additional weeks notice for every year of service up to a maximum of 12 weeks
