CALL TO ORDER:

The meeting was called to order at 9:35am.

REVIEW AND APPROVAL OF MINUTES:

The minutes for May 2014 call meeting were voted and approved by the committee.

OLD BUSINESS:

-Shekinah Glory’s policies & procedures for the use of mechanical restraint for medical/protective purposes have been approved since May contingent on Regional Advocate, Mr. Daye, receiving the requested corrections to the policy. Corrections have not yet been received, and Shekinah Glory will be added to the agenda for next meeting.

NEW BUSINESS:

-Family Preservation Services (Norfolk and Eastern Shore Regions) notified committee of affiliation of ABA OPT License under existing license (informational purposes only)Regions will rewrite letters using correct terminology and submit to Mr. Daye.

ADVOCATE REPORT:

Mr. Reginald Daye reported the following:

-Problems with logging on to or technical difficultieswithin the CHRIS system need to be addressed through the Delta Vita help desk.

-Quarterly reports and CHRIS reports are to be received by committee members 2 weeks prior to meetings, as stated in cooperative agreement. Reports should be mailed to members to avoid potential HIPPA violations. Reports should be faxed to Mr. Daye. Ms. Edwards has also asked that reports be faxed to her, rather than mailed, at 282-2990.

-Agencies who are not yet licensed will not have access to CHRIS.

-If a substitute attendee plans to participate in quarterly meetings, two copies of the cooperative agreement should be brought to meeting and signed by chair.

-General vacancy exists; candidates may not work or volunteer for agency.

-Advise employees that if applying for positions with other agencies that current agency’s documents should not be used as sample. This creates risk of HIPPA violations.

EXECUTIVE SESSION:

A motion was made and passed ULHRC enter into Executive Session pursuant to Virginia Code Section 2.2-3711 (A) for the protection of privacy of individuals and their records in personal matters not related to public business, namelyissues involving CMJ Positive Change, Essential Independent Lifestyles and VOA.

EXIT EXECUTIVE SESSION:

The LHRC voted to return to open session.

Upon reconvening into public session, each member certified that the only items discussed in closed session were:

  • CMJ Positive Change-advised to seek OT/PT eval for patient’s gait problem
  • Essential Independent Lifestyles-term “monitoring” needs to be explained explicitly to staff.
  • VOA-Any issue involving swelling leg needs immediate medical attention; policy in writing for medical evaluation if head involved.

FOIA TRAINING 2014:

Ms. Haqq provided 2014 FOIA training to the following LHRC members: Samuel Moore, Rosemarie Hughes, and Antonio Eley
