If a Lake Mary student would like to invite a non-LMHS student to be his/her guest at Prom, then this form must be submitted for administrative approval priorto ticket purchase.

Important to note:

• Only one guest will be permitted per LMHS student. Persons over the age of 21 or in middle school will not be permitted to attend.
• No refunds will be issued.
• Monday, March 7th is the last day to submit the guest form. Should a non-LMHS student fail to follow this procedure, he/she will be denied entry.
• Form must be submitted via fax or email to Mrs. Vanessa Marshall (AP) at 407-320-9512 or
• Faxes endorsing Non-LMHS students must be sent from their own school and may NOT be sent by a parent
• If the guest DOES NOT attend school, please provide a copy of a government-issued ID and this form to the attention of Mrs. Marshall in the LMHS front office.
• Approved guest list will be posted outside Mrs. Burke’s room (1-166) after school on Tues. March 8th and updated each day of ticket sales (March 9th-March 15th) in the auditorium lobby

I understand that any violation of these rules could result in my removal from the dance and the issuance of a trespass notice. FOR GUESTS ENROLLED IN A HIGH SCHOOL OTHER THAN LAKE MARY:TO BE COMPLETED AND SIGNED BY AN ADMINISTRATOR FROM GUEST SCHOOL

FOR GUESTS NOT ENROLLED IN ANY HIGH SCHOOL:

You must attach a clear copy of your valid driver's license or another form of government-issued photo identification to this form and turn it in to the front office at LMHS.
