NOTICE INVITING RE-TENDER


ANNEXURE-A

Sealed tenders under single bid systems are invited from the eligible contractors for providing services listed under scope of work in the bid document. The details of which are as follows:-

INSTRUCTION TO THE TENDERERS ANNEXURE-B

1. Tender form shall be completed in all respect.

2. Last date of submission of tenders is 16/08/2016 up to 1:00 P.M.

3. Date and time of opening of tender is 16/08/2016 at 2.30 P.M.

4. Each and every page of the tender documents should bear the stamp and signature of the authorized representative/quoting firm. It must be filled without exception.

5. The rates shall be quoted in Figure and words. In case of any discrepancy in rates, the rates written in words shall prevail.

6. The envelope containing tender document shall be sealed and bear the name of work and the name and address of the tenderer.

7. The work site that is ESI Dispensary No.3, Focal Point, Near Fire Brigade Office, Ludhiana can be visited on any working day during office hours by contacting Administrative Officer/Engineers / Care Taker/ General Branch/A.E. (Civil) Mobile No.9915300510 in order to have Idea of Work.

8. The Competent Authority of ESIC, reserves the right to accept or reject any tender or all tenders without assigning any reason whatsoever.

9. Conditional tenders are liable to be rejected.

10. The tender for the work shall remain open for acceptance for a period of sixty days (60) from the date of opening of tender.

11. These instructions shall form a part of the contract document.

12. Award of Work: -

(i) The selection of the agency will be at the sole discretion of the ESIC who reserves its right to accept or reject any or all the proposals without assigning any reason whatsoever.

(ii) Upon evaluation of offers the notification on award of contract will be intimated to the successful tenderer.

13. Duly filled Tender Document shall be deposited by hand delivery in the Tender Box kept in Dy. Director Office, Room No.202, 2nd Floor, Administrative Block, ESIC Model Hospital, Bharat Nagar, Ludhiana on or before 16/08/2016 upto 1.00 PM.

14. Delayed /late tenders for whatever is the reason will summarily be rejected.

SPECIAL CONDITIONS OF CONTRACT

ANNEXURE-C

1

1.  Performance guarantee will be 10% of the total of the bid, which will be in the form of Demand Draft and same will be deposited by the contractor within 7 days of award of contract. The Demand Draft will be made in the favour of ESI Fund A/c No.1 payable at Ludhiana.

2.  The material/components used for the work should be ISI marked of reputed brand and the ISI mark should be clearly visible on the material/components.

3.  Any material/component found defective should be replaced free of cost to the satisfaction of ESIC Model Hospital, Ludhiana authorities failing which the cost will be deducted from /Performance Guarantee furnished by the selected vendor.

4.  Item’s one year Warranty or Guarantee should be provided by the agency to the ESIC Model Hospital, Ludhiana in case of any problem in functioning of the item the either contractor will repair the same of it will repaired by ESIC Model Hospital, Ludhiana and the cost will be recovered from the performance guarantee. The performance guarantee will be released after 01 year of the completion of the work.
