SCHOLARSHIP APPLICATION

The Lassiter High School PTSA will award four $1,000.00 scholarships to deserving seniors who have exhibited outstanding community service and volunteer efforts.

Please answer all three questions and attach FOUR COPIES of your essay to this application. Please omit your name from each essay question.

1. What have you done during your high school years to give back to your community and what have you learned about yourself in the process?

Please describe in detail the types of volunteer work and community service in which you have been involved. The quality (impact) and quantity (time consuming, initiative) of your efforts will be considered. Also explain in detail anything positive or negative that you have personally learned about yourself as you have served others.

2. Below are the 6 Values of PTA. Choose one and describe what you have learned about it and how you have demonstrated this value during your time at Lassiter.

Return this completed application to the Counseling office no later than March 28, 2017. Applications will NOT be accepted after that date or if left in an alternate location. Do not put your name on your Essays. Please be sure to attach FOUR (4) copies of your essay answers to the completed application and place in a sealed envelope marked to the “Attention of PTSA Scholarships”. In addition, students are required to submit EITHER a letter of recommendation, OR the name and phone number of a reference who can validate the student’s volunteer efforts.
