High School students must submit the completed application to their Guidance Office
by 2 p.m. on Monday, April 2, 2018.
College students must submit the completed application to the chair of the Scholarship committee postmarked by Monday April 2, 2018
ELIGIBILITY:
In order to participate in the LAKELND MUSIC CLUB SCHOLARSHIP COMPETITON, students must:
_____1. Submit a completed application form.
_____2. High School Division - Be a graduating senior attending a Polk County High
School or studying with a Polk County teacher.
College Division – Be a freshman, sophomore or junior music major attending a
Polk County College or studying with a Polk County teacher.
_____3. High School Division - Have a 3.5 weighted GPA or better.
College Division – Have a 2.75 GPA or better.
_____4. Submit an official transcript with a school seal.
_____5. Include a personal letter giving relevant information about applicant.
_____6. Submit a letter of recommendation from a teacher in his/her field.
_____7. Submit a letter of recommendation from someone other than a family member.
_____8. Agree to perform at a Lakeland Music Club meeting on either April 10 or May 8, 2018.

High School Students: All applications and required attachments must be received in your Guidance Office NO LATER than 2:00 p.m. on Monday, March 26, 2018.
