Lake County 4-H Hall of Fame Award

The Lake County 4-H Advisory Committee is looking for 4-H Volunteers who go above and beyond in their position of a 4-H Volunteer. The Committee is looking for 4-H Volunteers who have excelled in the program in one or multiple ways for 10 or more years. The application should focus on the volunteer’s whole 4-H experience, not just specific to one year.

Guidelines:

·  The nominee is required to have 10 or more years of service.

·  The application is due to OSU Extension, 99 East Erie Street, Painesville, OH 44077 by June 29, 2017. NO late applications will be accepted.

·  This application must be completed by a 4-H Advisor, Committee Chair, Key Leaders, Parents, or a 4-H member. Nominees are not limited to nominating someone from their own club or project area.

·  Volunteers cannot nominate themselves. Application can be typed or neatly printed and all parts of the application must be complete to be accepted.

·  The winner will be announced at the Outstanding Youth Ceremony at the 4-H building immediately following the parade, on the first day of fair.

·  The inductee will also be nominated to represent Lake County for the State 4-H Hall of Fame.
