INFORMATION REGARDING HARBOUR FISHING

Your son/daughter/ward has selected HARBOUR FISHING for their sport on Wednesday afternoons. Listed below are rules and requirements that students must follow.

·  Students are to meet in the Quad at 1.30pm. Rolls will be marked and students walked to The Harbour along the designated route.(See reverse of this note for route map).

·  Students will be dismissed at 2.45pm from the venue or walked back to school to catch buses.

·  Students are to follow the instructions of the teachers-in-charge at all times.

·  Students are to have their own fishing tackle and equipment.

·  Mobile phones and ipods are not part of the sport session.

·  Students are not to interact with members of the public who are not part of the school sport group.

·  No student is to enter the water for any reason.

·  In the event of wet weather, alternative activities and areas may be used in and around Kiama HS. Listen for instructions; check the PE Noticeboard on sport day to confirm arrangements.

·  Students must wear appropriate footwear and full Kiama High School PE uniform. A cap or hat is strongly recommended.

·  Students must be free of injury to participate in this sport.

·  Infectious Diseases (“Blood Rule” Procedure) applies should an injury occur.

·  Students are to be aware of rules and safety procedures. Dangerous behaviour will not be tolerated.

·  Students should use sun protection, e.g. an SPF 30+, broad spectrum, water resistant sunscreen reapplied regularly.

·  Students should ensure that they have an adequate level of hydration before, during and after Sport. It is recommended that all students have a drink bottle with them during this activity, and drink regularly during each session.

·  Students requiring an early leavers pass to leave during the sport session must present a note from their parents to the front office before roll call. Students will be issued with a gate pass at this time. This note is to be shown to their sports teacher during roll marking at the beginning of the sport session.

·  The permission note below must be returned to your roll call teacher before you will be permitted to attend sport. Therefore, return it ASAP.

I hereby give consent to my son/daughter/ward, ______of class ______, to participate in HARBOUR FISHING commencing at school for Roll Marking and walking to Kiama Harbour for Sport.

My child has special needs of which you should be aware: (eg. Allergies, diabetes, medication etc)

To the best of my knowledge, he/she has no medical condition, disability or injury, which puts him/her at risk when participating. In the event of illness or injury, I authorise the seeking of such medical assistance on my behalf that my child may require.
