Bio:Albert C. Hergenroeder, MD, graduated from the University of Pittsburgh School of Medicine, Pittsburgh, Pennsylvania, in 1980 with a Doctorate of Medicine. He completed his pediatric residency in 1983 at Duke University Medical Center in Durham, North Carolina. In 1986, he completed his fellowship in Adolescent Medicine and Sports Medicine at the University of Washington, Department of Pediatrics, Seattle, Washington. He did his post-fellowship training at the Center for Sports Medicine in San Francisco, and then was recruited to Baylor College of Medicine.

Dr. Hergenroeder developed the Adolescent Medicine and Sports Medicine programs at Texas Children's Hospital starting in 1986. He has authored 97 articles and chapters. He is the fellowship director for the ACGME certified Adolescent Medicine Fellowship. He has been a team physician at two local high schools and two colleges. He is the Program Director for the annual Chronic Illness and Disability Transition from Pediatric to Adult Based Care, CME conference which will hold its 13th conference this year.

Dr. Hergenroeder is Professor of Pediatrics, Adolescent Medicine and Sports Medicine Section, Baylor College of Medicine and Chief of Adolescent Medicine and Sports Medicine at Texas Children's Hospital.

Bio: Becky Birchmeier, RN, MS-Nursing, is a pediatric nurse specialist working for the Marshfield Clinic Institute for Quality, Innovation and Patient Safety (IQIPS). She received her graduate degree from the University of Wisconsin-Madison in nursing education and her undergraduate nursing degree from Viterbo University in La Crosse, WI. She started her nursing career with an associate degree in nursing from Wisconsin Indianhead Technical College in Ashland, WI. Her professional experiences include obstetrics, public health and nursing education.

Bio:Darci is a pediatric nurse practitioner and faculty associate at the AFCH Cystic Fibrosis Center. She has been working with patients and families with CF since 1989. Darci, along with Craig, have been speaking on the topic of transition since 2005 both nationally and internationally. They have nearly 50 years of combined experience in working with patients and families with chronic illness.

Bio:Dr. Bob Rohloff has been practicing general pediatrics since 1984 at Southwest Pediatrics in New Berlin, Wi where he lives with his wife. He has four adult children. He became involved in the Wisconsin State Medical Home Learning Collaborative in 2004. Since that time he has been working to implement medical home components of care at CMG Southwest Pediatrics. The effort began with the creation of a Parent Advisory Board, which continues to meet every six weeks. . The Board has been responsible for advising and guiding the practice. As a result of this collaborative effort the practice has implemented several programs including: a community resource center in the practice’s lobby, an ADHD evidence based case management program, an asthma care management program, quarterly information meetings providing lectures on topics ranging from ADHD to autism treatments and internet safety, care coordination and care plan creation by staff nurses, an ADHD parent support group lead monthly by the practices collaborating psychologist and a quarterly newsletter electronically delivered to over 5,000 recipients. The newsletter provides information on community resources, practice resources, medical topics and parent perspectives. Parents and community members author many of the articles. The practice is actively partnering with the Children’s Hospital of Wisconsin’s Quality Improvement office, Children’s Research Institute and the Special Needs Center of Southeastern Wisconsin and the New Berlin Public Library-Children’s library. The practice has been recognized for its work by the Center for Community Inclusion at Utah State University. The practice will also be part of an upcoming AAP monograph on best practice in medical home care. Dr. Rohloff was named a Milwaukee area health care hero and received the Wi State Medical Society’s Physician Citizen of the Year award in 2012.

Dr. Schwake is a board-certified pediatrician who started private practice in July 2005. He and his wife have three children and live in Wauwatosa, Wis. He was actively involved with the CYSHN Transition Collaborative through Children’s Hospital of Wisconsin. He has also contributed to a statewide initiative to increase awareness and utilization of developmental screening among primary care providers.
