Windham Mountain Ski/SB Trip – January 4, 2014
Dear Parent/Guardian:
This is our first trip to Windham Mountain!! We saw the start of winter today and the snow will only keep coming!!
Enclosed is the information that you will need prior to the trip. Please read everything carefully and feel free to contact me any time if you have any questions or concerns. You can reach me at (908) 851-6570. Trip reservations will be given out on a first come first serve basis.
Here are the prices that will be charged for each group member. I will break it down so you know where the money is going. Please know that it is MANDATORY for beginners to take a lesson!!

We will be taking a coach bus to the mountain. The bus will leave Kawameeh parking lot at 6:00 am SHARP!! NO EXCEPTIONS!! BE THERE BY 5:45 READY TO GO!!!!! WE WILL LEAVE WITHOUT YOU!! I know it’s early but the kids can sleep on the bus and we’ll arrive at the mountain in time to make first tracks.
PAYMENT METHOD: CASH or MONEY ORDERS made out to cash only PLEASE! Please send your money in with all signed permission slips and waivers in a sealed envelope with your child. Be sure to label the envelope with his/her name and homeroom#. Money will be accepted starting at 7:30 in ROOM 25 the day after the trip is announced!

I am familiar with and understand the conditions of participation and understand that immediate and constant supervision are not part of the skiing/riding experience. I agree that in the sole judgment of Kristen Hudson, or any other chaperone, if my son/daughter violates those conditions, he or she may be sent home immediately at my sole expense, and I hereby represent and warrant that I will be responsible for any expenses so incurred. In the event that my child,
______, requires any medical, dental, surgical care and/or hospitalization, I hereby authorize and appoint Kristen Hudson or any other chaperone, to approve or disapprove, in my absence, any surgery, transfusion, anesthesia, use of any medicines or use of any medical procedures indicated or prescribed with respect to my son/daughter and to execute and perform anything whatsoever necessary or convenient and proper to be done in the premises. I understand that the aforementioned leaders will attempt to contact me with all reasonable speed if emergency treatment is necessary. However, this is no way diminishes the authority given to the herein.
In consideration of my/our child’s participation in the trip, I/we agree to the following in regard to the educational trip.
1. I agree to waive any and all claims and actions against Kristen Hudson, Passaic Valley Bus Company, the Union School District, its Board of Education, its employees, representatives, and agents that may arise as a result of the trip.
2. I agree that any claim for death, loss, damage, liability, accident, or of any nature whatsoever caused in the whole or in part by the act, omission, or negligence of any other person(s) shall not be brought against Kristen Hudson, Passaic Valley Bus Co., the Union School District, its Board of Education, its employees, representatives, and agents.
3. I agree that any financial obligation including but not limited to any cost, expenses and/or fees or liabilities that I/we must incur shall be my/our responsibility and I/we agree to indemnify and hold harmless Kristen Hudson, Passaic Valley Bus Co., the Union School District, its Board of Education, its employees, representatives, and agents.
