JR. HIGH AFTER SCHOOL PROGRAM
I , (parent/legal guardian) give permission for my child
, to attend the FAST after school program every Friday, which is sponsored by Loma Rica Community Church. I give permission for a representative from the church or a parent of jr. high students to give rides to my child from outside the gates of Foothill and Browns Valley Schools to David Rice's home in Browns Valley for FAST and to the church after FAST each week. We will be leaving Browns Valley every week by 4:40 pm and will be at the church by 5 pm., in time for the youth center to start. We will also be making small detours some weeks for things like getting ice cream at Collins Lake, buying a soda at the store, or checking out Dry Creek. We will always be back to the church by 5 pm. This permission slip also gives a representative of Loma Rica Community Church permission to seek medical attention for your child in case of an injury or accident in the course of our activities at FAST for the entire 2016-2017 school year. In signing this permission slip you are giving your child permission to participate in the program throughout the school year. We will begin on 9-09-16.
