The Primary Care Team: The Future is Now!

Featuring prominent experts from R.I. and around the country to discuss the future of primary care. Guest speakers will focus on the primary care team’s role in patient engagement, management of high-risk patients, behavioral health and more.

Objectives: At the end of the session, learners will be able to identify:

How the primary care team can be used to better support patient engagement

Strategies for meeting the needs of complex patients through the medical neighborhood

Strategies for providing integrated behavioral health solutions in primary care

This continuing nursing education activity was approved by the Rhode Island State Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.
