Dear John A. Pillar, Esquire:

The purpose of this Letter is to advise you that the above referencedtransfer application has been reviewed and approved by the Pennsylvania Public Utility Commission (Commission). However, before it begins operations, you must file with the Commission all of the information listed in paragraphs (a)and (b) below. You cannot operate under the approved motor carrier rights set forth in this Letter until all of the information listed below is filed with, and approved by, the Commission. Once the information listed below is received and approved by the Commission, you will receive a Certificate of Public Convenience,with PUC No. A-6417885,which authorizes you to begin operating under the motor carrier rights set forth in this Letter.

(a)An acceptable Form E filed by an insurance company which is evidence of bodily injury and property damage liability insurance. Your insurance company must file a Form E with the exact name of the applicant as it appears on this Letter – Hometown Transport Services, Inc., t/a Mt. Pleasant Yellow Cab, and t/a Jeannette City Transit. You should also advise your insurance company to place the following numbers at the top of your insurance form – A-6417885 andA-2015-2494832.

(b)An acceptable tariff establishing just and reasonable rates. Instructions for filing of a tariff can be found at: Instructions.pdf

(c)A vehicle list to include verification all vehicles have been properly titled to the business and the required registration has been obtained as required by 52 Pa. Code §29.101.

Upon issuance of a Certificate of Public Convenience you are granted the right to operate as follows:

To transport, as a common carrier, by motor vehicle, persons in paratransit service:

  1. Between points in the boroughs of Mt. Pleasant and Donegal, and the townshps of East Huntingdon, Mt. Pleasant, and Donegal, Westmoreland County, and from points in said boroughs and townships to points within an airline distance of twenty (20) miles of the limits of the borough of Mt. Pleasant, and return.
  1. Between points in the borough of Scottdale, Westmoreland County, and from points in said borough, to points within an airline distance of twenty (20) statute miles of the limits of the borough of Mt. Pleasant, Westmoreland County, and return.
  1. (1) Between points in the township of South Huntingdon, Westmoreland County.

(2)From points in the township of South Huntingdon, Westmoreland County, to points in Pennsylvania within an airline distance of fifty (50) statute miles of the limits of the city of Monessen, Westmoreland County; Limited to service for federal, state, county, or local agencies under programs sponsored by said agencies.

  1. (1) Between points in the city of Jeannette, Westmoreland County, and within an airline distance of three (3) statute miles of the limits of said City.

(2) Between points in the Borough of Delmont, that part of the Township of Salem within an airline distance of one (1) statute mile of the limits of the Borough of Delmont, and that part of the Township of Salem lying on the west of Pennsylvania Highway Route 66, and from points in said territory to points in the city of Jeannette, the Borough of Penn, points within an airline distance of five statute miles of the limits of the City of Jeannette, the Borough of Adamsburg, the Borough of Sutersville, and the Township of Swickley, all in Westmoreland County and return.

(3) Between points in the City of Jeannette, the Borough of Penn, that part of the Townships of Penn and Hempfield within an airline distance of one (1) statute mile of the limits of the City of Jeannette, and the Borough of Adamsburg, and from points in said territory to points in the Borough of Delmont, that part of the Township of Salem within an airline distance of one (1) statute mile of the limits of the Borough of Delmont, that part of the Township of Salem lying on the west of Pennsylvania Highway Route 66, the Borough of Sutersville, the Township of Sewickley, and points within and airline distance of five (5) statute miles of the limits of the City of Jeannette, all in Westmoreland County, and return.

(4) Between points in the Borough of Sutersville and the Township of Swickley, Westmoreland County, and from points in the said territory to points in the Borough of Delmont, that portion of the Township of Salem within an airline distance of one (1) statute mile of the limits of the Borough of Delmont, that part of the Township of Salem lying on the west of Pennsylvania Highway Route 66, the City of Jeannette, the Borough of Penn, points within an airline distance of five (5) statute miles of the limits of the City of Jeannette, and the Borough of Adamsburg, and return.

Rights D: 2, 3, and 4 subject to the following condition:

That no right, power or privilege is granted to provide service from the Boroughs of Manor and Arona, Westmoreland County, except for the transportation on return of passengers transported to the Boroughs of Manor and Arona by the applicant;

(5) From points in the Borough of Sutersville, Westmoreland County, to points in the Borough of West Newton, Westmoreland County, and return.

(6) Between points in the Township of Salem and the Borough of New Alexandria, Westmoreland County, and from points in said territory to points in the City of Greensburg, Westmoreland County, and from points in said territory to points in the City of Greensburg, Westmoreland County, the City of Jeannette, Westmoreland County and within an airline distance of three (3) statute miles of the limits of the said City, the Westmoreland Mall, in the township of Hempfield, Westmoreland County and the Green Gate Mall, in the Township of Hempfield, Westmoreland County, and return.

(7) Between points in the Boroughs of Murraysville, Export and Delmont, Westmoreland County, and from points in the said Boroughs to points within an airline distance of five (5) statute miles of the limits of said boroughs, and return

FAILURE TO COMPLY WITH ANY PROVISION OF THIS LETTER WITHIN SIXTY (60) DAYSOF THE DATE OF THIS LETTER WILL RESULT IN THE DISMISSAL OF THE APPLICATION AND REQUIRE THE FILING OF A NEW APPLICATION AND FILING FEE.

If you have not previously submitted a copy of a current satisfactory safety rating from the U.S. Department of Transportation or another state with safety regulations comparable to Pennsylvania, you must demonstrate safety fitness by completing a Safety Fitness Review which will be scheduled by a PUC enforcement officer within 180 days of the date your certificate of public convenience is issued/entered. Failure to submit to the Safety Fitness Review will result in the cancellation of your certificate. You should become familiar with the requirements of 52 Pa. Code as applicable to the operation of a common carrier as authorized by this grant of authority. Any change in address of Hometown Transport Services, Inc.must be reported to the Commission by filing a Change of Address Form. This form can be found on the Commission's website at:

/onlineforms/pdf/MC_Address_Change.pdf. Failure to comply with all applicable requirements may subject the carrier to penalties, including fines, suspension of operating rights or cancellation of authority. Title 52 of the Pennsylvania Code may be accessed at .

In particular, we urge the applicant to become familiar with the regulations at 52 Pa. Code §29.333(e) Vehicle Age and 52 Pa. Code §29.503 Age Restrictions. Effective January 1, 2008, vehicles operating in limousine service may not be more than 8 model years old, unless otherwise permitted by the Commission. A common or contract carrier may not permit a person to operate a vehicle in its service unless that person is at least 21 years of age.

If you are dissatisfied with the resolution of this matter, you may, as set forth in 52 Pa. Code §5.44, file a petition with the Commission within twenty (20) days of the date of this Letter.
