Jobtitle:Occupational Therapist JOBPURPOSE

•ToprovidespecialistOccupationalTherapyassessmentoftheneedsofpeoplewithdisabilitiesandthroughrehabilitation,re-ablement,information,advice,equipmentand/oradaptations,enableidentifiedneedstobemet,aimingtoimproveoutcomesfor people in thecommunity.

•Workaspartofacommunitybasedteamtoprovideanoccupationaltherapyservicetopeople,meetingtheirneedsinthemostappropriateplace/setting.

•Autonomouslymanageacaseloadofcustomerswithcomplexneeds,utilisingevidencebasedcustomerfocussedprinciples.Useanoutcomebasedapproachtoplan,carryoutassessment,intervention,andevaluateOTinvolvement.

•WorkinanintegratedmannerwithcolleaguesfromotherteamswithinSocialCare,Housing,HealthandthirdsectororganisationstoensurethatcustomersreceiveanOccupationalTherapyserviceproportionatetotheirlevelofneed.

•WorkaspartoftheOTserviceinPembrokeshire,whichaimstopreventdependencyand protect the vulnerable. The County Council work closely withtheLocalHealth board toensureanintegratedapproachtothewholeOTServiceacrossSocialCare,Housing,Health,Children’sServicesandEducation.Aimingtodeliveranefficient,highquality,customer focussedservice.

PRINCIPALRESPONSIBILITIES

1.UndertakespecialistOccupationalTherapyassessmentforeligiblecustomers,who arecurrentlylivinginthecommunityorarereturningtoliveinthecommunity.

2.Basedonassessment;planandprovideappropriateoccupationaltherapyinterventions, based on best practice and evaluateoutcomes.

3.Undertaketelephoneassessments,providinginformationandadvicetocustomersandcarerstoresolveimmediateproblemsandgatherinformationtosupportappropriate referral pathways to occupationaltherapy.

4.Prioritise allocated referrals, utilising agreed prioritisationprocedure.

5.Manage allocated caseload effectively andsafely.

6.Provideprofessionaladviceandsupporttocolleagueswithinandacrossagencies.

7.WorkinpartnershipwithotheragenciesandcolleagueswithinSocialCare,Housing,Healthandthirdsectororganisationstobreakdownorganisationalbarriers forcustomers.

8.Participateintraining,CPD,undertakeauditandsupportappropriateresearch.

9.Support the induction and training ofothers.

10.Maintainacurrentknowledgeoflocalpoliciesandprocedures,guidanceandlegislationthataffectsoccupationaltherapypracticeandensurecomplianceofownpractice.

11.Providesupportand/orsupervisiontoOccupationalTherapyAssistantsasandwhenrequired.

12.Supportoccupationaltherapystudentplacements,whicharepartofgainingskillsregarding social services and housingfunctions.

13.Contributetothesettingofobjectives,targets,prioritiesandstandardsfortheservice.

14.EnsurethatinformationsystemsincludingCareFirstareaccuratelymaintainedanduptodateinordertoadheretoprofessionalrecordkeepingstandardsandprovidedata that will inform servicedelivery.

15.Carryoutanyotherdutiescommensuratewiththegradeasdirectedbythedesignatedmanager,orasamutuallyagreeddevelopmentopportunity.

PERSON SPECIFICATION

1.DegreequalificationorequivalentinOccupationalTherapyandcurrentregistrationwith the Health and Care ProfessionsCouncil

2.PostregistrationexperienceofworkingasanOTinarelevantareaishighlydesirable.

3.Clear/concise/effective written and verbal communicationskills

4.Ability to self organiseand manage acaseload.

5.Ability to assess, balance and managerisks.

6.A strong customer focussed valuebase.

7.Understandingoftheeffectsofdisabilityandillnessoncustomers,theirfamiliesandcarers

8.Currentknowledgeofrelevantlegislationandabilitytoapplyinpractice.
