DESCRIPTION FOR SCOPE OF WORK:
Excavations/Site-lighting
RECOGNIZED HAZARDS:
Slip and fall from loose soil, dirt, sand or rock, struck by equipment
HAZARD AND SAFETY CONTROLS TO BE IMPLEMENTED AND/OR PROVIDED TO PROTECT
OUR WORKERS FROM THE RECOGNIZED HAZARDS AND CONTROLS TO PROTECT OTHER
TRADES THAT MAY BE AFFECTED BY OUR SCOPE OF WORK:
Keep spoil minimum of 2 feet from trench, equipment to have back up alarm, flagman to be used if needed, ladders to be provided every 25 feet when digging 4 feet or deeper, excavation to be inspected daily by competent person prior to work being performed, barricades in place to protect other workers, earth support or sloping will be done, protection of underground utilities.
TRAINING:
Weekly toolbox meetings to be conducted along with daily pre task meetings. Ongoing training to be conducted as needed. Retraining to be provided to employees as necessary so that the employees maintain the needed understanding and knowledge required.
DISCIPLINARY PROCEDURES:

OUR WORKERS FROM THE RECOGNIZED HAZARDS AND CONTROLS TO PROTECT OTHER
TRADES THAT MAY BE AFFECTED BY OUR SCOPE OF WORK:
GFCI protection and all electrical cord and plug sets tested and documented prior to use, when damage is evident, before equipment is returned to service following repairs and at times not exceeding 3 months. Extension cords to be listed for "heavy or hard use." All cords and equipment are to be visually inspected daily for external defects. Circuits to be properly identified, all unused openings sealed, all covers in place, lock out / tag out to be used to protect all employees, lamps to be protected from breakage, extension cords not to obstruct traffic, proper clearances to be maintained around equipment, all energized work to follow proper procedures. All power tools to be grounded or double insulated. Unsafe tools to be tagged and removed from service.
TRAINING:
Weekly toolbox meetings to be conducted along with daily pre task meetings. On-going training is to be provided. Retraining is to be provided to all employees as necessary to maintain knowledge and understanding.

Material to be kept clear of walkways, material to be kept 10 feet from the edge of the building, normal housekeeping will be maintained to eliminate trip or fall hazards.
TRAINING:
Weekly Toolbox meetings will be done along with daily pre task meetings. Proper lifting techniques training to be provided to each employee along with retraining as necessary to ensure employees maintain the required knowledge and understanding.

Electrical and Fire Alarm Rough
RECOGNIZED HAZARDS:
Fall from ladders, trip hazard from material, lacerations from metal jacketing, metal framing, electrocution or shock from temporary power, eye protection.

OUR WORKERS FROM THE RECOGNIZED HAZARDS AND CONTROLS TO PROTECT OTHER
TRADES THAT MAY BE AFFECTED BY OUR SCOPE OF WORK:
Safety glasses and hard hats are to be used at all times. Material is to be stored in a neat and organized manner. Hand protection to be used, extension cords to be inspected on a daily basis, proper lighting to be provided, housekeeping measures will be maintained within the work area.
TRAINING:
Weekly toolbox meetings will be done along with daily pre task meetings. Retraining will be provided for each employee on an as needed basis.

OUR WORKERS FROM THE RECOGNIZED HAZARDS AND CONTROLS TO PROTECT OTHER
TRADES THAT MAY BE AFFECTED BY OUR SCOPE OF WORK:
All unused openings will be sealed. Circuits will be properly identified, all covers will be in place, flexible or extension cords should not created trip hazards, proper working clearance around equipment will be maintained, all energized work follows proper procedure, all grounding will be properly installed and terminated, all employees will wear proper PPE, lock out tag out procedures will be followed to protect all workers. Work site will be kept neat and orderly, electric rooms will be free of debris and scrap, proper signage will be in place, access will only be given to qualified persons, safety glasses will be worn at all times.

All equipment and electrical systems are to be properly grounded. All grounding conductors are to be properly protected from physical damage. Grounding system is to be tested to verify zero potential. Grounding conductors are to be bare copper or green in color only, for identification purposes.

OTHER TRADES THAT MAY BE AFFECTED BY OUR SCOPE OF WORK:
Hoisting and rigging is to be performed by qualified persons. Fall protection and tag lines are to be used as required, flagger to be used as needed, safety glasses to be worn at all times. All unused openings will be sealed, circuits will be properly identified, all covers will be in place. Flexible or extension cords should not create trip hazards, proper working clearance around equipment will be maintained, all energized work will follow proper procedure, all grounding will be properly installed and terminated. All employees will wear proper PPE, lock out / tag out procedures will be followed to protect all workers. Work site will be kept neat and orderly, electric rooms will be free of debris and scrap, proper signage will be in place and access will only be given to qualified persons.

OUR WORKERS FROM THE RECOGNIZED HAZARDS AND CONTROLS TO PROTECT OTHER
TRADES THAT MAY BE AFFECTED BY OUR SCOPE OF WORK:
Ladders will be inspected daily, prior to use for damage, tagged and removed from service as required. Areas around the top and bottom of ladders will be kept free of debris, step ladders will only be used in the open position. The top or top step will not be used for standing or sitting, ladders shall be properly secured, only fiberglass or wooden ladders to be used. Material is to be kept out of walkways to minimize trip hazards. Trash and debris are to be cleaned daily. Fall protection to be used as required, fall protection to be inspected daily. Proper lifting techniques to be used when moving materials. No material to be stored within 10 feet of the leading edge of building. Eye protection is to be worn at all times.
TRAINING:
Weekly toolbox meetings will be done along with daily pre task meetings. Ongoing ladder and fall protection training will be conducted. Retraining will be provided to employees as necessary so that the employee maintains the knowledge and understanding required.

OUR WORKERS FROM THE RECOGNIZED HAZARDS AND CONTROLS TO PROTECT OTHER
TRADES THAT MAY BE AFFECTED BY OUR SCOPE OF WORK:
Proper personal fall protection to be used when required, fall arrest systems to be inspected daily before use, safety glasses to be worn at all times. All equipment to be visually inspected before being energized, circuits to be properly identified, all covers to be in place, all energized work to follow proper procedures, lock out / tag out kits to be used to protect all workers. Proper working clearances to be maintained around all equipment, proper signage to be posted, and access will be granted only to qualified personnel. Electric rooms are to be free of scrap and debris and work site is to be neat and orderly.
