JOB TITLE: Wellbeing Coordinator

MAIN PURPOSE OF THE POST

To work as a member of the integrated neighbourhood/extended practice teams, (based in various GP Practices) establishing referral pathways to preventative and wellbeing services and activities. To support individuals to build personal resilience in self managing their health and wellbeing through assessment and review of individual needs, development of individual wellbeing plans, and motivating behavioural change.

DUTIES AND RESPONSIBILITIES

Service Operation

  1. To receive referrals and assess the needs of individuals, facilitating access for them to recommended brief interventions and programmes of activity that benefit their wellbeing
  2. To support people within the community and their own homes to develop and implement well-being plans
  3. To use health improvement information to help people to self-manage their health and encourage behavioural change, including people with long term conditions
  4. To support individuals to recognise and change their current lifestyle and to identify how their way of life might affect their health and well-being
  5. To help individuals navigate services and activities that are available to them in their community and make informed choices
  6. To be respectful and non-judgemental of people's lifestyles, and to be able to encourage them in choices and actions that are acceptable and achievable to them
  7. To be aware of and sensitive to barriers to adopting a healthier lifestyle such as affordability, accessibility and life circumstances.
  8. To use motivational interviewing techniques to encourage people to engage with opportunities for physical and social activity, and to encourage and motivate people, using creative ways of addressing negative behaviours
  9. To enable clients to make choices and develop a wellbeing plan to change behaviour and sustain those changes, leading to more healthy choices and actions
  10. To support individual wellbeing plans over an agreed timescale and review and revise wellbeing plans with individuals as appropriate
  11. To maintain a high level of confidentiality in all aspects of work including client records and conversations and maintain comprehensive records of clients of the service
  12. To work to agreed quality and performance frameworks

Development and Promotion

13.To build networks with local community resources such as activities and services that may have an impact on health and wellbeing

14.To develop multi-agency working across communities and voluntary and statutory organisations to improve health and address health inequalities

15.To work with colleagues to embed a culture of prevention within integrated neighbourhood/extended practice teams, providing support to the team around health improvement

16.To identify means of development of the service to improve the health and wellbeing of the local community and identify barriers that individuals are experiencing in changing their behaviour

Administration and Accountability

17.To ensure the completion and maintenance of all appropriate administrative records

18.To gather and collate statistical and other information and data as required, to ensure effective qualitative and quantitative monitoring and evaluation

19.To report as required on activity and outcomes

20.To attend individual, team and staff meetings as required

Other

21.To develop an awareness of local and national developments and best practice in this area of work, and to attend relevant conferences, meetings and training events as required

22.To ensure that all work is undertaken in accordance with the aims and values of the agencies involved and in accordance with relevant policies

23.To work flexibly and positively with the various agencies involved

24.To support the fundraising activities of the charity

25.To undertake any other relevant duties as required from time to time by the line manager, Head of Service or Chief Executive

PERSON SPECIFICATION

Essential:

  • Excellent interpersonal skills
  • Good written & verbal communication skills with awareness of the need for confidentiality at all times
  • Good listening skills with the ability to adopt appropriate style and method of communication
  • Ability to recognise mental and physical health problems and identify when further advice is required
  • Ability to work to tight deadlines
  • Ability to travel freely across Cheshire
  • Good organisational skills including information, resources and time management, and ability to establish and manage systems and processes and self motivated
  • IT skills, including word processing and databases
  • Understanding of the role preventative services play within Health and Social Care
  • Knowledge of the local area, networks and communities
  • Understanding of motivational interviewing techniques
  • Knowledge of health improvement interventions including managing long-term conditions
  • Experience of working with the public

WELLBEING COORDINATOR - JOB DESCRIPTION

Additional Information

Age UK Cheshire is working with West Cheshire CCG GP Practices to introduceWellbeing Coordinators to work as part of the new integrated neighbourhood/extended practice teams. This role will be based between the “Princeway” cluster of GP practices (which includes; Helsby, The Rock, and The Knoll) establishing referral pathways to preventative and wellbeing services and activities in the community. The job holder will support individuals to build personal resilience in self- managing their health and wellbeing through assessment and review of individual needs, development of individual wellbeing plans, and motivating behavioural change. There is a wealth of literature and evidence regarding the use of brief interventions, motivational interviewing, and health coaching and the pilot is informed by NICE Guidance and Quality Standards and Pathways. The evidence shows improved outcomes in physical health, mental and emotional wellbeing, and reduced demand on health and social care services.

Hours :29 hours per week, worked over 4 days, ideally Tuesday to Friday. Occasional evening or weekend work may be required in order to attend promotional events / meetings.

Salary : £20,885 pa

Travel: The nature of the job will require travel across Cheshire. A full driving licence and use of a car is essential. Travelling expenses will be paid for travel incurred in the course of duty (paid at Inland Revenue dispensation rate – currently 45p per mile).

Holiday Entitlement: Five weeks leave p.a. (excluding Bank Holidays) rising after 5 years continuous service by one day per year (pro rata) to a maximum of 6 weeks leave after 10 years.

Contract: All new staff are subject to a six month probationary period. Four weeks’ notice is required to terminate employment by either side i.e. the employee or Age UK Cheshire. During the probationary period, one week’s notice is required by either side.

Funding: Current funding for this post is in place until March 2017 and continuation of the service after this date will be subject to securing further funding.

Pension: Age UK Cheshire has an automatic enrolment workplace pension scheme in place for eligible employees.

Health Care: After the 6 months probationary period staff will automatically join the Age UK Cheshire’s Health Care Cash Plan and will be entitled to receive healthcare benefits.

Childcare Vouchers: After the 6 months probationary period staff will have the option to make use of the Age UK Cheshire Childcare Voucher scheme (available to eligible staff whilst in receipt of salary).

Criminal Disclosure: The post holder will be subject to police checking by the Disclosure and Barring Service (was CRB) Enhanced Disclosure + check of Adults Barred List.
