JOB TITLE: Substance Misuse Nurse (Bank)

Responsible to: Senior Nurse/ Head of Operations for Wales

Location: Dyfodol – Barry, Bridgend, Cardiff, Merthyr Tydfil, Neath, Pontypridd, Swansea

Hours: Zero fixed hours- to be agreed on a rota basis

Salary: £13.30 an hour

Background and Service

Kaleidoscope is a pioneering substance misuse treatment provider, offering evidence based treatment and holistic support to individuals who access its services. Staff teams work collaboratively with partner agencies including G4S and WCADA (Welsh Centre for Action of Dependency and Addiction), National Health Service, Public Health Wales,, CRC (Community Rehabilitation Company) and Her Majesty’s Prison and Probation Service to deliver different aspects of these services, with a core aim of engaging substance misusers in their recovery goals and support them throughout their treatment journey. For individuals who require substitute opiate prescribing treatment, clinical and nursing services are provided by Kaleidoscope, with prescribing interventions being undertaken by Sessional GPs who have a special interest in Substance Misuse (GPwSIs). .

Clinical services include substitute prescribing for opiate dependence, harm reduction initiatives including the provision of take home Naloxone; family planning and sexual health screening; Blood Born Virus testing and referral on to specialist support. . All clinical services are overseen by KaleidoscopeClinical Director , with substance misuse nurses supporting the effective delivery of these services.

Service users will be offered a full range of support in order to assist them in overcoming their substance misuse.. Treatment will be individualised via personal care planning and regular clinical reviews, and will seek to support service users to address their substance misuse whether opiate, stimulant, hallucinogen, Image Performance Enhancing Drug (IPED),_alcohol or other Novel Psychoactive Substance (NPS) as well as to raise motivation to address wider social and health issues.

Substance Misuse Nurses willdeliver services in line withWelsh Government Treatment Frameworks, NICE guidance (National Institute for Health and Clinical Excellence), and other relevant professional standards and legislation. Kaleidoscope staff are expected to follow Operational Policies and Standard Operating Procedures (SOPs),ensuring that the care and treatment provided is safe,undertaken to a high standard and informed by up-to-date, evidence-based practice.

The post holdershould enjoy face-to-face contact with people who have recognised that their use of substances has become problematic. S/he will have excellent communication skills, be able to work as part of a multidisciplinary team, across agency cultures including those within statutory, private and voluntary sector partnerships, andThe post holder must be flexible and efficient within small locality teams, to ensure services are delivered equitably and with the service user at the heart of delivery.

S/he will have a non-judgmental approach to people who have substance misuse problems and be able to provide a welcoming and accessible service to existing and potential service users.

Individual supervision will be provided six weekly in line with Kaleidoscope policy. Additional clinical support will be provided via peer supervision and review groups. Specific training will be available for the development of service provision in line with Continued Professional Development.

Dyfodol is commissioned by the South Wales Police and Crime Commissioner, and Her Majesty’s Prison Probation Service to act as a dedicated response to substance misuse service users involved with the criminal justice system. G4S are the lead contract provider and services are provided in line with local Substance Misuse Area Planning Board delivery plans.

Dyfodol aims to provide rapid access to substitute opiate prescribing treatment and wrap around support alongside psychosocial interventions and harm reduction initiatives. Dyfodol actively encourages service user involvement throughout the delivery and development of service provision. Dyfodol provides comprehensive substance misuse assessment and onward referral for service users entering and leaving custody, arrest referral support and ongoing community supervision orders. Dyfodol staff will support individuals to address substance related issues, access appropriate interventions, reduce physical dependence and related health problems,reduce drug and alcohol related offending and improve physical, psychological, family and social functioning.

The post-holder will be integral in the successful completion of clinical assessment, treatment recommendations, delivery and review of statutory orders such as Drug Rehabilitation Requirements (DRR), Alcohol Activity and Alcohol Treatment Requirements. You will also provide interventions to assist with the delivery of test on arrest scheme and community supervision orders and post prison licencing conditions.

The post holder will be expected to work in line with the Social Services and Well Being (Wales Act) 2014, and the Violence Against Women Domestic Abuse and Sexual Violence (Wales) Act 2015 to ensure safeguarding of individuals (adult and children) is at the heart of service delivery. A good understanding of confidentiality and information sharing legislation is required with the confidence to act professionally and seek guidance and support from line management and colleagues where appropriate.

Key duties

The Substance Misuse Nurse will:

  • Provide advice, information and guidance on the management of substance misuse to clients engaged with services.
  • Undertakecomprehensiveclinical assessments
  • Assess individuals for DIP/DRR intervention and provide court and probation reports as necessary
  • Carry a caseload of clients being prescribed substitute opiate medication or awaiting ` alcohol detoxification.
  • Agree treatment plans with clients, and review them regularly
  • Undertake drug screening
  • Ensure case notes and other statutory records are up to date and kept in accordance with Kaleidoscope policies and procedures / regional information sharing protocols
  • Deliver clinical interventions, including HCV testing, HBV immunisation and naloxone training/administration.
  • Liaise with Community Pharmacies to ensure prescriptions are in placeand monitored appropriately.
  • Provide advice, support and training to GPs, pharmacists and other professionals where appropriate.
  • Work within the parameters of the NTA’s substance misuse community prescribing

guidelines (2007). Deliver onsite dispensing of substitute opiate medication and

provide support and cover for service users accessing community pharmacies when required.

  • Participate in clinical audits and service reviews.
  • Facilitate the transfer of clients from secondary to primary care services, and lead with referrals / liaison with Community Mental Health Teams (CMHT’s) and other community substance misuse treatment providers on leaving the criminal justice system.
  • Work with other regional treatment services to support the development ofthe local treatment system, and in particular Shared Care services
  • Actively encourage service user involvement and feedback and ensure views are considered in informing the development of the service.
  • Collate and enter data electronically, and contribute to service reports

Person Specification

PERSON SPECIFICATION:
Essential / Desirable
Qualifications and training / RGN/RMN with valid NMC Pin. /
  • One year post-registration experience in Substance Misuse
  • Independent/supplementary nurse prescribing qualification
  • Training in Anaphylaxis and BBV
  • Relevant training in Phlebotomy
  • Relevant specialist qualification in alcohol screening and brief advice and substance misuse treatment.

Experience/ Knowledge / Excellent case management skills, with experience in undertaking comprehensive clinical assessment, care-planning and treatment reviews with people who have diverse and complex needs.
Experience working with substance misusers. / Experience of treating individuals requiring alcohol detoxification.
Experience of developing and maintaining successful partnerships with GPs, treatment agencies and other healthcare providers.
Experience in undertaking clinical audits.
