JOB TITLE: Senior Case Manager, Napa

REPORTS TO:Program Coordinator/Case Manager

EMPLOYMENT STATUS:Non-exempt

TIME COMMITMENT:Full-time, 40 hours/week

STARTING SALARY:$18.00 to $20.00 per hour

SUPERVISORY RESPONSIBILITIESVolunteers 2-5

Position Summary

The Senior Case Manager supervises the Rainbow House day-to-day program activities of the clients, as well as provides counseling and case management. This position coordinates resources and services in cooperation with the Program Coordinator, area agencies and clients, and develops pertinent self-sufficiency workshops for clients.

Essential Functions

  1. Assess client needs, establish goals with clients, assist with financial and housing strategies, and monitor client follow-through. Provide guidance, assistance and resource information to clients; ensure application for available services/public benefits, and client follow-up. Advocate for clients as appropriate. Work with individual clients in a counseling atmosphere to address issues and stresses in their lives.
  1. Ensure the quality and maintenance of documentation, case files and on-going program statistics; provide timely written reports to immediate supervisor.
  1. Read and provide essential information concerning clients in the daily shift reports and/or clients’ files to assist in the on-going support of clients by other staff members.
  1. Assist in facilitating the program to be in compliance with Council on Accreditation (COA) standards. Be thoroughly familiar with the policies and procedures guiding the work of this position in particular and the program or department overall. Perform job functions in line with these policies and procedures.
  1. Coordinate job and educational opportunities in cooperation with client’s vocational counselor.

  3. Be an example of behavioral expectations. Be a positive role model and representative for Catholic Charities in the community.
  4. Maintain a professional appearance and presence.
  1. Work with Program Coordinator to prepare monthly schedules for House Aide staff. Prepare alternate coverage plans or cover for these employees when necessary due to illness, vacation, or emergencies.
  1. Develop new program components as directed and provide on-going education classes on related housing topics.
  1. Represent agency at community meetings as appropriate and as assigned.
  1. Participate in training required by accreditation standards, plus performance and quality improvement efforts.
  1. NOTE: Catholic Charities considers this position to be a mandated reporter of elder and child abuse.

Other Responsibilities

  1. Perform other related duties as assigned.
  1. Attend all in-service trainings and participate in leadership building.
  1. Attend meetings, including case management, as assigned by supervisor.

Education, Experience, and Skills Required

  1. MSW preferred; a bachelor’s degree in social work, psychology, sociology or closely related field, along with internship or work experience with homeless youth, required.
  1. Experience and or certification in conflict resolution, harm reduction, smoking cessation, wrap-around model, substance abuse, financial literacy or other recognized relevant Life Skills programs.
  1. Demonstrated experience in working with the homeless or low income families. Sensitivity to issues surrounding families experiencing homelessness, substance abuse, mental illness, or physical handicaps.
  1. Ability to work under pressure. Experience in crisis intervention and problem solving with ability to diffuse situation without heightening the conflict.
  1. Ability to co-lead an educational workshop with Program Coordinator.
  1. Exhibit the core competencies, habits, critical thinking, attitudes, behavior, and drive to help make Catholic Charities and the Rainbow House Program a center of excellence and a model program in our community and the nation.
  1. Ability to collect and report on data required for grants and other funding sources.
  1. Interviewing and counseling skills.
  1. Strong written and verbal communication skills.
  1. Knowledge of recovery programs, such as the 12-Step Programs, and local resources preferred.
  1. Bilingual (English/Spanish) highly desirable.
  1. Have working knowledge of community resources.
  1. Cooperative, friendly, and helpful attitude with clients and co-workers. Ability to work closely with other employees to ensure positive, constructive environment within the program or department, and throughout the agency
  1. Ability to thrive in a flexible, fast-paced and growth-oriented environment, while maintaining a sense of humor and a positive, solution-oriented approach.
  1. Passion and enthusiasm for the mission of CCDSR and its clients.
  1. Ability to work independently and in a team setting.
  1. Must pass fingerprinting clearance prior to start of employment.
  1. Valid California Driver's license, DMV record check and proof of insurance.

Physical Requirements

  1. Requires ability to sit from 3 – 6 hours per day with intermittent occasional walking, standing, bending, squatting, and climbing.
  1. Occasionally may be required to lift items up to 50 pounds waist high.
  1. Occasionally may be required to carry items up to 50 pounds for distances up to 30 feet.
