JOB TITLE: Clinical School Based Social Worker

CLASSIFICATION: Exempt (10-month position)

DEPARTMENT: Mental Health Services

QUALIFICATIONS: Requires LMSW from accredited graduate school of social work.

BASIC FUNCTION: Under the supervision of the Clinical Supervisor, the Clinical Based Social Worker will provide mental health services to children and their families. These services will be provided on-site at school satellite location, focusing on diagnoses and utilizing a variety of treatment modalities.The Social Worker will also provide case management through linkage referrals as needed, maintain appropriate documentation of all services rendered, and will effectively collaborate with school staff at satellite location. This is a 10-month position (September-June), that will follow the Department of Education Calendar for time off.

Job Responsibilities:

  • Provides mental health services to a caseload of children/adolescents in individual, group, collateral, and family modalities.
  • Provides these mental health services with an awareness of, and respect for, diversity of culture, ethnicity, gender, and sexual orientation. This knowledge shall be incorporated into therapeutic practice.
  • Accepts referrals from PPT team, school social workers, guidance counselors and whom ever the principal deems appropriate and initiates contact as appropriate.
  • Master and utilize computerized treatment systems (AWARDS) for all aspects of client record-keeping/charting, toward assuring all documentation is maintained in comprehensive, organized, and timely manner.
  • Coordinates intake evaluations: scheduling of psychiatric evaluation, writing psychosocial summary, determining initial DSM V diagnosis, assessing need for psychiatric referral.
  • Write daily progress notes, initial treatment plans, and quarterly treatment summaries for all individuals on caseload.
  • Develops (with clients) measurable short-term and long-term treatment goals with action plan and completion time frame on a quarterly basis.
  • Helps individuals on caseload (and their parents) access appropriate entitlement benefits and needed resources/concrete services.
  • Establishes and maintains cooperative relationships with school staff and community partners of the school as indicated.
  • Maintains individual case records for caseload, assuring that necessary reports are in place, all documents are filed appropriately.
  • Maintain required LOS, submit weekly log and monthly LOS report
  • Completes discharge summary and completes case record documentation for those clients who discontinue services in an appropriate timeframe.
  • Participates in weekly case conferences.
  • Participates in weekly supervision (s).
  • Maintains cooperative and collaborative relationships with co-workers.
  • Participates in community-building activities sponsored by program and agency and encourages participants to support these activities.
  • Fulfill all documentation required by Department of Education for funding purposes.
  • Reports to a Clinical Supervisor
  • Other duties as assigned.
