Job Title: CareLink Officer

Based at: Saltbox Offices, Hanley

Salary: £21,000 (pro-rata)

Contract: Fixed Term 6-8 weeks

Hours: 28-35 hours per week

Responsible to: CareLink Team Leader

Responsible for: Supporting the development of a high quality CareLink service for older people

Summary of Duties

A.  To provide a high quality customer focussed Older persons service

B.  To undertake home visits to clients and prospective clients

C.  To work as part of the team who are developing CareLink

D.  To encourage clients to maintain independent living and healthy lifestyles

Main Duties

Core Services:

·  To receive and administer CareLink referrals and complete the necessary documentation as required

·  Complete and maintain all client contact information, manually or electronically as appropriate

·  Resolve or redirect client queries by liaising with staff/agencies/organisations

·  Assist with the planning and delivery of events

·  Conduct satisfaction surveys with a range of stakeholders as required

·  Provide support and advice to CareLink volunteers

·  To undertake an initial assessment home visit to outline the CareLink Service ensuring the necessary documentation is completed accurately and appropriately

·  To work with client to assess their needs and produce a support plan agreed with

·  To identify with clients a range of options to meet their identified needs

·  To be responsible for co-coordinating the agreed support plan and where appropriate, to implement and evaluate agreed actions including direct work with clients

·  To work with clients providing direct advice and assistance as required

·  To ensure that there is a full and comprehensive documentation trail to support the development of the project

·  To work as part of the team to explore the development of CareLink

·  To support the CareLink Team in developing a volunteer team to work within the CareLink service

·  To be part of a rota to provide on call cover at weekends and Bank Holidays as and when required

General:

·  To work collaboratively as part of the CareLink team, with partners, communities and people of all faiths and none

·  To monitor and co-operate with the evaluation of the work of CareLink

·  To provide the line manager with regular reports and assessments

·  To investigate and report ways in which the service could be developed and/or improved

·  To undertake such other duties as may be required by your line manager and/or the Saltbox Management Team which are commensurate with the position

·  To work within the organisations professional boundaries

·  To adhere to the policies and procedures as specified by the Saltbox Trustees

Skills / Knowledge / Experience / Experience of working with vulnerable adults and have an understanding of the challenges that older people face / Ö
To relate to and empathise with older and vulnerable adults / Ö
To have a basic understanding of vulnerable adult
policies and strategies / Ö
Possess and demonstrate good written and oral communications skills / Ö
To have a working knowledge of services and agencies available to support vulnerable adults / Ö
Experience of multi partnership working is desirable / Ö
Developed organisational, administrative and monitoring skills / Ö
Most have good time management skills.
Must produce accurate and timely work with attention to detail / Ö
Experience of monitoring and evaluating projects against targets and outcomes / Ö
Experience of gathering information, analysing and then providing user-friendly summaries of information / Ö
Must possess good IT Skills: / Ö
