Responsibilities:

Capabilities /
  • Manage an assigned case load to support the commencement and implementation of supports in the participant’s NDIS Plan; including informal, mainstream and community, as well as funded supports that need assistance with implementation.
  • To monitor and review the spending and quality of services and programs in the participant’s NDIS Plan.
  • To provide assistance to strengthen participant abilities to connect to and coordinate informal, mainstream and funded supports in a complex service delivery environment.
  • Complete a Service Agreement with the Participant and their family before the commencement of the service, which defines the communication and partnership agreement that will be implemented to reach decisions regarding services they choose, resolve service delivery issues, and address barriers to their participation.
  • To support the participant to reach a Service Agreement with their “funded” support providers identified within the NDIS Plan.
  • Liaise and work professionally with NDIA Planners and Local Area Coordinators, health and other support providers as relevant to gather and verify information to facilitate plan implementation.
  • Conduct support needs and risk assessments for participant’s with complex support needs, interpreting complex information about functional limitations, personal factors and putting supports in place to assist their successful participation in the community.
  • To participate in the formal review of the participant’s NDIS Plan when requested by the participant and/or as specified by the NDIA Planner.
  • Ensure high quality record keeping and provide NDIA with reports on outcomes and success indicators within the agreed reporting frequency.
  • Ensure that the National Disability Insurance Scheme Act 2013 legislation is maintained in accordance to the role.

  • Demonstrated understanding of the National Disability Insurance Scheme Act 2013.
  • Awareness and appreciation of current prevailing philosophies and trends in the area of disability support services.
  • Knowledge and capability in managing case loads
  • Capacity to work effectively both as a sole worker and as a member of a multi-disciplinary team and adapt quickly to a changing environment.
  • Excellent interpersonal and communication skills including the ability to initiate and maintain contacts with a broad range of service providers. High capability in relationship building.
  • This Diverse problem solving skills and a high degree of original and independent thinking. Decisions should reflect broad practice covered by both MOIRA and NDIA precedents and policies. Specifically for NDIS Support Coordination, sound judgement and decision making in relation to accountability of funds and interactions with individuals and communities.
  • Demonstrate an understanding and knowledge of disability and its impact on individuals and have a positive and contemporary attitude to people with disability. Specifically for the NDIS Support Coordination, competency in applying assessment tools to identify support needs.
  • A commitment to achieving positive outcomes for NDIS Participants, the Scheme and for MOIRA, utilising planned results to ensure and measure customer-focus and satisfaction.

Qualifications / Possess a Tertiary qualification in the community sector or other relevant discipline and capability in working with people with disabilities, their families and carers.
