Job Purpose

The post holder will lead and coordinate the VOICESsurvey on behalf of the hospice and in collaboration with the Tower Hamlets Clinical Commissioning Group. They will work in partnership with multiple care providers operating in Tower Hamlets to improve for people approaching the end of life.

Aims

  • Using a validated, nationally recognised questionnaire, collect carer/relative feedback on the care delivered in the last months of life
  • Using data gained from the VOICES questionnaire identify areas of unmet needs, differences in care provided by different services or to people with different conditions, and their carers.
  • Identify areas for improvement in provision or coordination of services and use this evidence to plan further service developments in end of life careusing existing routes and pathways ensure survey results are fed back to all relevant stakeholders to improve future care delivery

Background

  • The service has been commissioned by the Tower Hamlets Clinical Commissioning Group (CCG), to be provided by St Joseph's Hospice.The project involves work across a number of care settings including the hospice, hospital services and community servicesfor a fixed period of time until April 2019.
  • The post holder will be based at St Joseph’s Hospice but will connect with both hospital and community health care sectors forming key working relationships with stakeholders in each organisation.
  • The views of informal carers for the evaluation of service (VOICES) survey is a validated tool to assess the quality of care delivered to people in the last 3 months of their lives (1)

Responsibilities

  • Be responsible for the quality of the work produced
  • Work across organisations to connect the VOICES work and output from the VOICES survey
  • Ensure that all day-to-day survey related liaisons runs smoothly
  • Prioritise and review workload as required
  • Ensure the VOICES questionnaire is reproduced accurately and represents all organisations
  • Retrieve death and next of kin reports from stakeholder organisations
  • Auditing and cleansing of these reports as well as importing data into database to organise mail outs, manage responses, interviews and focus groups as well as record enquiries and complaints.
  • Working with the 24/7 Help and Support Line team to follow up calls, help participantscomplete the survey, arrange support and sign-post to other services as necessary
  • With support from the Volunteer Manager recruit ad-hoc volunteers to support mail-outs and data input.
  • Organise, coordinate, conduct and analyse interviews and focus groups
  • Recruit, train and oversee other interviewers as required and in coordination with line manager
  • Supervision and training of volunteers/advocates/interpreters during their period of engagement with the survey
  • Be responsible for own supervision and accessing support following difficult conversations with the bereaved.
  • Manage all stages of from resource management, to mail outs, responses, analysis and reporting.
  • Prepare and be responsible for all mail outs, inclusive data, documents and survey packs
  • Manage project database and record all project activities
  • Manage the analysis, interpretation and reporting process so that the project reflects the CCGs respective objectives
  • Coordinate annual dissemination reports among stakeholders and participants
  • To ensure that the project is resourced and implemented correctly, to the satisfaction of the respective CCGs
  • Manage and analyse qualitative and quantitative data from survey, interview and focus group
  • Write detailed annual reports
  • Liaise with the CCGs and healthcare providers to provide regular reports and updates on the project performance, feedback, trends, and risks.
  • Contribute towards discussions around long-term sustainability of this project across multiple CCG areas
  • Respond flexibly to changes and challenges affecting the project, e.g. working with new stakeholders, such as community groups and changing care providers
  • Manage change of stakeholders and key relationships.
  • Manage and anticipate risks that may affect the project.
  • Manage and reorder survey documents such as letterheads, survey booklets

Managing the project:

  • Ensure that the service contractis kept up to date and regularly reviewed to ensure the service meets the expected outputs
  • Set upand lead a steering group with key stakeholders to guide and support the project
  • Develop a project plan with milestonesto be shared with members of the steering group
  • Manage a small budget to cover ad-hoc expenses incurred in the course ofthe project
  • Ensure milestones are met, and where not possible highlight these issues to the Clinical Governance Lead, and the Nurse Consultant for the Community Palliative Care Team.

Communication and partnership

The post holder will actively establish and develop relationships with other managers and teams within each organisation to promote the project

  • Manage, organise and record minutes of bi-monthly steering group meetings
  • Ensure an open, regular communication between all organisations to keep them informed of progress and any delays incurred.
  • Demonstrate a political awareness of the different needs and issues facing both organisations.
  • Possess a proactive approach that encourages participation in decision-making processes.
  • Work with the clinical governance and information governance leads in each organisation to ensure access to robust and quality data and is compliant with NHS information sharing standards.
  • Regular communication with clinical leads

Professional Development andTraining

  • Take responsibility for own professional and personal development which will be monitored in monthly management supervision sessions and within the annual appraisal system.
  • Adhere to the policies and conditions of service of St. Joseph's Hospice relating to sickness and absence, conduct, Health and Safety, information Governance and Data Protection and any others that are relevant
  • Keep updated with developments within the CCG regarding patient and carer experience of care and the national Voices survey
  • Access the staff supervision programme as required

Reporting

  • Produce annual reports, and statistics for the respective CCGs in accordance with project timelines and outputs:
  • At least 50 completed surveys per year.
  • At least 20 to 30 interviews and 2-3 focus groups per year.
  • Provide regular project performance reports to CCG, steering group and line manager.
  • Report risks and data trends to CCG, steering group and line-manager as required.
