Overview

Wirral based charity WIRED are part of a multi-organisational framework that delivers Early Intervention and Prevention Services to vulnerable adults and children across Wirral. Within the Adult element it is a requirement to support service users currently within primary care, secondary care and health and social care provision in Wirral to access community and voluntary based health and wellbeing provision within Wirral in order to facilitate positive outcomes for the service users in relation to their health and well being. The role of the third sector connector will be to liaise with Health and Social Care professionals to identify, support and connect vulnerable adults to services and activities in the community that will encourage community support, independence and improved health and wellbeing. The following skill areas are essential for this role;

  • Knowledge and understanding of Early Intervention and Prevention based services in Wirral
  • Robust knowledge of community and voluntary sector health and wellbeing sector providers and services available
  • Experience of case management within service provision
  • Experience and use of monitoring and feedback processes

Objectives

Wired is a registered charity that provides a range of services to disadvantaged people and carers within Wirral, Liverpool, Warrington & Knowsley.The objective of the Third Sector Connector is to, as part of the Early Intervention and Prevention contract, liaise with Health and Social Care professionals to identify, support and connect vulnerable adults to services and activities in the community that will encourage community support, independence and improved health and wellbeing.

Tasks and Responsibilities

  • To liaise with professionals in designated ICCT to identify and support vulnerable adults who could benefit from connections to community groups and organisations.
  • To support services users for a short period of time and identify a clear exit strategy so that service users can move into other services on a long term basis.
  • To work closely with Health Professionals within the Integrated Gateway, POPIN, First Contact and Neighbourhood Teams to identify those needing immediate support following a hospital stay, and those service users with more complex needs that will require longer staff involvement.
  • To attend regular meetings with other Third Sector Connectors, identifying any trends in referrals across the Wirral and feeding this into monitoring reports.
  • To carry out a full risk assessment and ensure that any risks are managed and minimised as soon as possible so that the service can be provided in a safe environment.
  • To provide a full explanation to clients and/or referrers if a service cannot be delivered and offer an alternative solution or contact where possible.
  • To liaise with colleagues in other departments of WIRED and also Wirral Health & Wellbeing CIC to maintain an awareness of services and activities to which clients can be directed.
  • To ensure that referrals are responded to within specified timescales.
  • To develop a good working relationship with external partners and community groups so that up to date information on community activities can be passed on to service users and professionals
  • To be flexible, person-centred and embrace changing approaches to delivery at all times in order to ensure that the Service is fully responsive to and accommodating of the needs, wishes and aspirations of the people who use it
  • In conjunction with Line Manager, to use a range of outcome monitoring tools to assess the impact of the Service on individuals on a quarterly basis
